You can't grapple onto the skylights on the Dark Carnival finale, the ones on the top of the stage at the front. Found that out the hard way.

You can grapple onto anything by holding down the fire button.

No, but I have set up diescraper so it works with the custom weapons regardless.

To make it spawn at the beginning of the map you would have to use cheats, or modify the scripts in the base mod.
