Try reloading Left 4 Dead 2

Solution B:

1. Go to Library - Games.
2. Right Click L4D2 - Properties
3. Go to Local files - Browse local files
4. Go to left4dead2/addons/workshop
5. Move 488536123.vpk to the folder left4dead2/addons
6. Reload L4D2, check the addon is enabled, which is now called Custom Name Change.
7. Enjoy.
