Use this when playing with both groups at the same time on survivor set 2. So having mixed l4d1 and l4d2 characters is also intended for this addon.

Should I use it only when playing 4+ survivors or I can have it for both one and two survivor groups?

Yes this is a very niche version specifically made for servers with both survivor groups at once. Server needs to use survivor set 2 (L4D2 survivors). Most of these servers already use this survivor set to match the names, UI icons and first person arms of the L4D2 survivors.
