Sound like you try in a 'Single Player' session.
Always set your server with 2 Player' atleast.
The single Player mode is kinda weird, I don't recommend it.

So, I've never heard of this, but I know that on some maps, the addon can have some rendering problems.
Try it on other maps to see if the problem persists (GM_Flatgrass)
I think you'll have to deactivate all your addons and find out which one is responsible for your problem.

The code can be found on the card supplied with the addon, in the same section as the briefcase containing the anomaly.

whenever my mind is dragged out of my body, it fades to black. it never un-fades from black when i return, i cannot see the eye room or the spinning skull, it's like i'm only rendering on-screen effects and models refuse to work. this happens in multiple addons and i am struggling to diagnose why

It's with a bot player, it's not an npc strictly speaking. He is therefore considered has a player. And then I periodically manipulate its movements. Otherwise, for your 'prank' there must be mods to do that, but in the case of mine, you must activate the case.
