The only thing I would like to complain about is that in my opinion the tainted gutworms have too little HP, so they can't really show themselves. It would be nice to increase their HP and/or defence by a factor of 2.
They should either get reduced health or spawn later in places like the depths/womb/sheol.
Not only normal Gutworms have too much health for their floor and level of danger, tainted variant can spawn in non Ascent floors and end your great runs. Having an enemy with around 130 health in Caves 1 is surely very balanced and fair.
