Would it be possible to isolate the "Monarchy to Republic" feature from the rest of the mod? I've been looking and it seems like this is the ONLY mod with this ability. Unfortunately I don't want to brick the AI or Invictus. I also don't want to use console commands and risk messing up my country.

No, all missions are standardized and generic. Some starts are better based on geography - for example, the Nile delta is still quite populous as is the Italic peninsula - and that will give you a bit of a leg up.

Is there more missions for certain 'areas' or nations? Are some starts better overall even though the idea is to level everything out, at least for mp?

@Bolty218 our map is the same as Invictus (although we might be slightly behind because they didn't finish their latest changes before we started more MP campaigns)

@firespark84 no, it will not work with invictus. this is an overhaul in many ways so it is incompatible. however, we work with some of the invictus modders, so the overall map is the same (except broken up)

@Stochastic_congruence This is a direct fork of the "first" HMO, necessary because the stewards of that version disappeared. We've tweaked or changed some of that stuff, but overall it's pretty similar. This version continue to see updates!

Wow some neat changes in this mod. Love the more character interactions! Is there any way these character interactions and foreign involvement actions could be a mod on it's own? Or incorporated into Invictus? Awesome stuff!
