If you pull down the mod itself and look at its internals, you may find it is not that hard.

Pick one of the 4 starts you like, click "Copy" and then use the up/down arrows to re-organize your starting priorities and click on the priority buttons (A), (B), etc to make changes in each category.
So, create a new template, click on the priority button such as (A) next to Experience and chance your profession to Doctor (or any other!)

only ever played unmodded; is there a step I am missing or a file that needs editing to get this to work? am I misunderstanding what it does?
The description says +2 for Fort, Wis, & Resilience but there is no bonus to Resilience. Also appears to be a +8 to bleed damage/defense(?) bonus added to the Mortality Known trait as well.
I'm learning things!
Random suggestion: Flavor text ("check the bandages") suggestions (things that physicians commonly say)
"Remember your ABCs" - refers to Airway, Breathing, Circulation
"They're not dead until I say they're dead" - Doctors are the only ones that can pronounce a patient dead in a hospital
"All bleeding stops eventually" - self explanatory...
