You are using it on a player who has died right? not just a ragdoll/npc? If you are, then I don't really know.

so i just crouch on a meduim range and hit left click?

If you are talking about the key used to respawn, it should be W. For the revive tool, it's just left click, however there are some conditions for it to work. One of them is that you must not be too close (to not get stuck), not too far away, and that you much crouch. The medpack is just left click and right click.
