Sam isn't technically a follower/companion. I didn't want to him to be affected by the follower limit so I just gave him an AI package to follow the player around and did other things to make him act like a companion. He doesn't use any of the stuff followers like Lydia use. (He's not part of the follower faction)
I even keep Sam around with me when I'm with Lydia. I really don't know what would be causing that issue, sorry.
like other people have said, you should look into adding more dialogue if you can, because you did an awesome job with what's there now. if he said some phrases during fights(other than the mudcrab battle), that'd be really fun. (i do like how he lacks idle chat though, as i have never liked humanoid or canine followers for that reason. i do love the little skeever purr sounds though.)
i did run into a small problem where he stopped responding to commands after a while. i reloaded and it seemed to fix itself, but i'm not sure if its a glitch or not.
