Report a translation problem
Just when you start Bloom, take Erritis, Tybir and Rhin into your party. Immediately rest (so Decanted shows up) and try to sell Rhin to Decanted. Erritis and Rhin should both hate you now.
Go to Aen-Tozon and feed him with Tybir. Then proceed with Tybir quest and always make rude comments ("we don't have time for that" etc) and in the end allow Tybir to kill Ragmin. Now everyone in the party should hate you.
However, later I got quite many reports from people who only managed to get the achievement if they gave away the scalpel. So my suspition is: at some point the devs modified how Memovira is achieved and now it's just buggy as hell. Or maybe it was always buggy. What works for some people doesn't seem to work for others.
- feed every Maw
- kill everyone you can (murdens, artaglio's crew, aggressive reflections, sunderman, Ioxu, waits-for-prey, memovira's patrol, kill the lascar crew). But, I didn't have to trick the patrol trapped in the Old Slave Block, I just let them go. I was also able to allow the catena's ghosts to live in my mind.
- don't use the Tides or scalpel on any Maws
- stole the cortex
- have skaydi give in
- absorb the memories of the dead at the Trade Post
I had to start a completely new playthrough for memovira unfortunately and this time i did literally everything you mentioned in the guide. in my opinion its by far the worst/hardest (e.g. i got despised easily on first try). you won't believe how much time i spent on the Memovira achievement xd. Downsides of being a completionist :D

As the Memovira, it is a strange one and I can't establish exactly what makes it tick. What works for some, does not seem to work for others. What is certain is that the Scalpel is very important here. Trading it in SEEMS to be necessary (judging by many, many reported experiences of various people), so maybe the fact that you could not do that is what is stopping your Memovira status.
The fact that the Maws are not attacking you is a very good sign indeed. It's been quite a while since I played the game, but I don't think there is any specific dialogue choices you have to make in the Heart. I'm quite sure I'd make a note of it.
