Basically just a quick copy of my forum post, detailing the achievements and how to unlock them, as well as a handful of secret/powerful/endgame items I've discovered =)

Secret Items

"Hero Armour"

Return to Adam once you have the airship, check Kellen's mother's grave to get ultimate defensive armour for Kellen

"Steel Curtain"

After finding Joe on Lone Island (south-west of Brashear) you'll be tasked with finding the First Mate of the Raiders. He's drinking in the pub in Borges. Speaking to him will net you this armour for Kellen, it has much less M.Def than the Hero Armour, but makes up for it by giving you 50% elemental resistence. Don't forget to return to Brashear to tell Joe's mom he's going to be okay ;)

"Fancy Dress"

Return to Borges once you have the airship, go into the docks area and the underground market. Speak to the old man near the market stalls to receive a powerful armour for Azzy - more defensively powerful than the Candy Dress, but without the +30 MAG stat

"Candy Dress"

Return to the very first Forest in the game, next to Adam, and go back to the crazy witch Lorabeth. She'll offer to make you a dress made out of magical candy, but is missing Magic Sugar. Return to Adam and go to the item store, check the crates to the right of the merchant to get the sugar, then return to Lorabeth. She'll finish the armour. It's incredibly powerful, though not as defensive as Fancy Dress, yet comes with a staggering +30 MAG stat. Lorabeth also mentions a Candy Cane that she lost on an island somewhere.....

"Strong Cover"

Incredibly powerful armour for Curtis, with great defensive power and +35 ATK stat. Return to Brashear and head to the west/left of town to find Shayne & Joe's house. Go downstairs and their mother will ask you to find Joe, whom Shayne marooned on an island to the south-west, Lone Island, and she'll give you the Strong Cover armour before you go.

"Miracle Badge"

Also in Borges, head to the east/left and re-enter the Pirate Cave. Head inside and check the chest in the Captain's old room to find this badge, which functions like a Ribbon and prevents all negative status effects.

You can find another Miracle Badge in a cave on an island, you are tasked with finding Mayah's brother, Anjal, and have to fight possessed thieves. Beating them (tough fight, come prepared!) nets you the second Miracle Badge.

"Fae Weapons"

Both are items for Talon. "Spearheads" are a set of super strong darts that have 100% hit chance, "Torchflame" is a set of armour with huge defensive powers. Return to Dradora once you have the airship, then go into the Den in the shady part of town. At the far north of the den, a headhunter friend of Talon's waits. He has two Fae weapons which you can only acquire by winning a game of chance, rolling a natural 1 on a 1000-sided die :D

Rumour has it you can acquire the Luck of the Fae, which would ensure you win any game of chance, by avenging the Fae people. Supposedly the Fae forests were reduced to a desert by a vengeful dragon........

"Lucky Darts"

Fairly strong darts with exceptional luck, giving +30 LUK stat, found at the back of the cave on the island where you rescue Mayah's Brother. Slightly less ATK than the Fae weapon Spearheads.

"Candy Cane"

Magical weapon for Azrael, much lower physical ATK compared to other Azzy weapons, but MAG stat is massively improved. Find this on Candy Island in the North-East, once you get the Airship, as a reward for defeating the hidden boss DisasterCake!

"Legend Sword"

Found inside the Lost Cave, a dangerous and difficult dungeon just to the right of Wind Tower. This is home to some of the more irritating sliding box puzzles in the game -- as well as, for now, the strongest and most annoying enemies too. The enemies hit hard, hit often, and hit fast, with "Mirage" being an especially (I'd say seriously overpowered, to be honest) enemy ability that casts Blur on the entire team.

Work your way through the dungeon, solving the puzzles as you go, to get some powerful gear. The Legend Sword will be amongst them and will be the most powerful sword in the game for Kellen.

"Titan Slayer"

This will be given after the short quest line that involves the brothers, Joe and Shayne, and their dispute over Titan Island and Raider Island. Each believes the island is shaped differently, either a T or an R. You'll find this island just north of Lone Island and it contains the brothers - Joe will send you to find his "Titan Slayer" sword from his ex-GF in the ruined town of Firmus, just next to the volcano range. Go there, enter the ruined tower and exit out of the right, head towards the small river and you'll see Tammy - a red-haired girl - who will give you the sword.

Return to disputed island and give it to Joe. A scene will happen and then you'll get the sword for Kellen. It's very strong but the Legend Sword from Lost Isle is more powerful.

"Raider Ripper"

Immediately after receiving Titan Slayer above, exit Joe's cave and head to the right and up, to enter the - now deserted - Titan camp. Head to the middle cabin and go around the back to enter a secret cave containing a purple chest and Raider Ripper, a staggeringly strong set of claws for Curtis.

"Bottomless Keg of Beer"

In the Borges pub there is an NPC in the corner who mentions a 'bottomless keg' of beer. This can be found somewhere in the Lost Cave, and he will reward you with 4x elixir if you return it to him.

"Quicksilver" / "Parity"

Additionally there are several secret rooms in the final dungeon, in which I found rare accessories such as 'quicksilver' (which enables your character to always move first) and 'parity' (which provides them with +20 ATK & MAG bonuses).

You might want to include the Juice Boxes and Essences, too. Or do a separate guide for each.

@moeffz Monk is found inside the Lost Cave near where the Legend Sword is found.

Found Monk somewhere. That is another armor for Curtis - in comparision to Strong Cover it has -35 ATK, -8 DEF, +50 M.DEF

Additionally Disaster Cake only pops up once Lorabeth told you about Candy Cane.
