Description
Nightmare is the 5th difficulty level and one step above expert (contains realism aswell).

Things that have been changed:

-non headshots deal less damage
-nearly all infected have sligtly increased health
-the tank has now 12000 hp instead of 8000
-commons deal 30 damage from the front and 24 damage from the back
-friendlyfire is 2x more dangerous (now you really have to be careful with guns and fire)
-most infected are sligtly faster
-some other small changes that are not worth writing them down seperately

I've never played L4D playing it first time in VR, seems too easy in VR hopefully this makes it hard, last 9hrs getting my VR experience ready so i'll find out tomorrow. Shame i can't jump in VR

is there anyway you can make a mod that gives only the tank incresed health by 2x? or spawn double tanks? that'd be cool if you could, or if thats already a mod, lmk

In case you mean how to access it: Its a mutation. This addon contains normal nightmare and nightmare realism.
In case you meant how to survive the campaign: Basically the same way as on expert just harder. You basically have to choice between playing expert realism until it feels somewhat easy to you or to try nightmare realism really hard.
