You need to S rank every missions (you don’t need to get s rank on the duplicated missions) if he doesn’t appear then maybe the game is bugged

You need to complete Ground Zeroes Mission
then upload your data from the main menu of MGSV Ground Zeroes then download the data in MGSV The Phantom Pain main menu

Yes you need to find the XOF patches on The Ground Zeroes Mission to unlock deja vu(solid snake Mission) and jamais vu(Raiden Mission).

you don't get raiden uniform from the second mission "jamais vu"
because you need to get S rank on all main missions on The Phantom Pain
