ZONE 1: The Forge Lands, The Weeping Crag (1 chest)

The Weeping Crag
Requires: Voidwalker/Phasewalker
Inside the same room as the GnoMAD gnome. To get to this room, there is a lift you need to use and once at the top, on the left will be a Voidwalker portal panel. Walk opposite of the portal panel and on the left will be a ledge. Look up across the ledge to find the second portal panel on the ceiling.
ZONE 1: The Forge Lands, The Nook (2 chests)

The Nook chest #1
Requires: Soul Splitter
Inside the big main room with all the doors, a well and a Boatman coin. Climb the wall to the upper floor.

The Nook chest #2
Requires: Voidwalker/Phasewalker
Inside the ball puzzle room with a ball and 2 elevators. At the top where the ball is supposed to go using the elevators, look for a balcony with a Voidwalker portal panel on the opposite side of the room. The first portal panel was at the bottom, just outside of the ball puzzle room. Entrance to this room is on the upper left of Bheithirs boss room.
ZONE 2: Kingdom of the Dead, The Breach (1 chest)

The Breach
Requires: Voidwalker/Phasewalker
Inside the room where you have to lift the 2 bridges. Lift the first one and step on it. Look up near the ceiling at the top left to find a Voidwalker portal panel. Climb the wall on the right of the bridge to find the second portal panel.

Teleport to Serpent's Peak, turn around and follow the path until it splits, then follow the path on the left to the end. Solve the puzzle to reach the chest.
ZONE 2: Kingdom of the Dead, The Spine (1 chest)

The Spine
Requires: Voidwalker/Phasewalker
Map Location: http://i.imgur.com/lsMev4g.png
Teleport to Judicator's Tomb. Facing the tomb entrance, turn left and follow the path which will shortly split. Stick to the right path and climb the tower. Once you're at the top, you should see a regular chest. Get to this chest, then walk to the edge of the platform and look down. There is a small hole that you must avoid. Carefully drop down, avoiding the hole. Turn around to find a Voidwalker portal panel. Step close to it and look up to find a hole in the ceiling. Through this hole you should see the second portal panel.
Death Tomb (4 chests)

Death Tombs
Requires: 4 keys from selling book chapters to Vulgrim.
Each Death Tomb contains 1 chest. They are in plain sight, so all you need to do is open it.
DLC/Additional Campaign, The Abyssal Forge (1 chest)

Additional Campaigns can be accessed on the Continue screen. Each additional campaign unlocks after completing a certain part of the story.

The Abyssal Forge
Unlocks after completing the main story in Kingdom of the Dead.
Outside where you spawn, there is a door on the north side which appears to be unreachable. Inside is a puzzle that will lead to where the chest is. Before you can get to it, you need to proceed south through the main dungeon to get the Mad Smith's Device from the Mad Smith. The device allows you to walk on the corrupted tar-like surface like Jesus.
DLC/Additional Campaign, Argul's Tomb (1 chest)

Additional Campaigns can be accessed on the Continue screen. Each additional campaign unlocks after completing a certain part of the story.

Argul's Tomb
Unlocks after completing the main story in Lostlight.
At the very beginning, where you find Ostegoth and he tells you a story, go left and look for a hidden caveway on the right side just before the big gate that leads to the main dungeon. Get to the end of the cave by either defeating enemies along the way or just skip them. At the end will be an arena where you will have to endure and defeat all enemies that spawn. After you defeat all the enemies, the chest will appear.
EXPLOIT
Each skill point chest is supposed to award you with a single skill point however you may have noticed that some chests actually gave you 3 skill points instead. That's because you opened the chest while using Soul Splitter. If you open the chests during Soul Splitter, you will receive 3 skill points instead of 1. This bug/exploit cannot be avoided with 2 of the 12 chests.

NOTE: If the exploit no longer works, it was most likely fixed in a patch.
Q & A
Q: I only have the base game, does that mean I can't get the DLC chests?
A: Darksiders 2 Deathinitive Edition comes with all the DLCs. They're actually called Additional Campaign in Deathinitive Edition but I still like to refer to them as DLCs since they used to be DLCs in the original Darksiders 2.

Q: None of the chests gave me any skill points. Why is that?
A: Skill point chests are only available in the Deathinitive Edition. In the original Darksiders 2, these are hidden chests that only drop random items.

Q: You said each chest only gives 1 skill point but I got 3 out of some of them.
A: The chests that gave you 3 skill points were opened using Soul Splitter which triggers the exploit I mentioned.

Q: Can I open the skill point chests again in NewGame+?
A: Yes.

Q: Is there also a chest in The Demon Lord Belial campaign?
A: No.

Q: There are supposedly 12 chests but you only have 8 videos?
A: The Death Tomb video contains locations of 4 chests and The Nook video contains 2 chest locations which brings the total to 12 chests.

So when do i look at this guide, I see the first chest requires voidwalker - at what point do i refer to this within the game, level or progress wise?

Thank you for these guides, very helpful. Sucks we don't have Soul Splitter for the Argul's Tomb chest, but if I remember correctly you have plenty of points for all skills by the end anyway.

Yeah, I've gotten the Weeping Crag chest just wall-running and jumping, too. The price of doing things the hard way :(

what if i told you, that i completely skipped the voidwalker on the weeping crag chest with some really, really precise climbing?
