How to set-it-up :D
1. Download and open HEX-Editor http://www.heise.de/download/4e6101013cc45f9a277158874f2547f2-1461011087-2253304.html

3. Open DarkSoulsIII.exe (Don´t forget to make a backup of the original File);

2. HIt Search (F3) and change the following Values (Search for Hex-Values)

ONLY For 2560 x 1080 Resolution! :

Find HEX string :: 800700003804

and change it to :: 000A00003804

--------------------------------------------------------------------------------------------------------------------------

3. Save and quit - Overwrite the Original DarkSoulsIII.exe with the new (Hex)edited one.

4. Go to C:\Users\YourUserName\AppData\Roaming\DarkSoulsIII and open GraphicsConfig.xml

Set your 21:9 resolution values in the following first Lines to ::

<config><ScreenMode>WINDOW</ScreenMode>
<Resolution-WindowScreenWidth>2560</Resolution-WindowScreenWidth>
<Resolution-WindowScreenHeight>1080</Resolution-WindowScreenHeight>
<Resolution-FullScreenWidth>2560</Resolution-FullScreenWidth>
<Resolution-FullScreenHeight>1080</Resolution-FullScreenHeight>

5. Save AND set the file to READ ONLY! <<<<-- IMPORTANT

6.Start Dark Souls 3 - Press ALT+ENTER to go to Fullscreen mode --->>> DONE!
Enjoy DS3 in glorious 21:9!

(Because we have to run ds3 initially in window mode, we have to set it to Fullscreen (ALT+ENTER) every time we start DS3)

"Martan" added some nice Infos how to swap to another Resolution - Thank you very much!

If you want to get any custom resolution in game - you can swap any existing resolution by this method above.

The example above is swaping the internal
1920x1080 (800700003804)
to
2560x1080 (000A00003804)
or
3440x1440 (700D0000A005)

For any custom resolution use this program - ResHex to get the right HEX strings for your desired resolution.

WARNING!

:::YOU COULD GET BANNED OR SOFTBANNED FOR DOING THIS, SO BE CAREFUL WITH ONLINE GAMEPLAY! :::

I TAKE NO RESPONSIBILITY FOR ANYTHING THAT WILL GO WRONG IF YOU´RE USING THIS

****EDIT 05.05.2016****
I´m playing Dark Souls 3 with this fix for ~2 weeks and didn´t get any Problems with Online gameplay. Did so with Dark Souls 2 and SOTFS too, also never run into any Issues. In my opinion using this Method is safe, but remember there might be a chance to get hit by the strange anti-cheat runtimes atm...
