This is a complete guide on all the game medals, objects and documents.

Introduction
Hydrophobia[en.wikipedia.org] was the game originally released for XBLA on September 2010, and later updated to Hydrophobia Pure on December 2010.

On May 2011, Hydrophobia: Prophecy[hydrophobia.wikia.com] was released on Steam. Later, on November 2011, it was also released on PS3. Although very similar to the Xbox version, some levels were changed, as well as the location of some of the collectables.

This guide was written and tested on Hydrophobia: Prophecy for Steam, code version 0032, compiled Jul 7 2011. It lists all collectables (objects, documents, hidden documents) and all medals.
Glitches
Sometimes, upon game start, the "Load game" main menu option is unavailable. If this happens, just restart the game (yes, this is the workaround suggested by the dev team). At times I have to restart it several times until the game decides to load the saved progress.

The Supersonic medal exists in-game, but is not listed in the in-game database.

The Explorer medal is glitchy. Sometimes it is not awarded, even though the player had found both rooms. Restarting the playthrough from the beginning and trying again should work.

The Deep Breath medal is glitchy and unfair. If the player manages to obtain both documents but die before going back to the surface, this medal will be unobtainable. Obtaining it will require wiping the game progress (which is stored at "steam/userdata/{numbers}/92000/remote/").
Don't screw up!
There are some medals with strict conditions, requiring restarting the entire playthrough if you screw up.

If you want to get the Geek medal, make sure you never fail a hack throughout the entire game. If needed, press the button to disengage the hack. Aborted hacks don't count as failure. If you screw up, restart the playthrough from the beginning. While hacking, it is easier to match the two wave signals if you focus your attention on the left-most region.

If you want to get the Supersonic medal, make sure you only use the basic Sonic Rounds as the weapon (or the water powers). You may temporarily use another weapon to aid getting another medal (as noted in Stealth medal), but make sure to reload from the latest checkpoint after you do so.

The Deep Breath medal may require wiping the entire progress if you screw up. Try practicing that section a bunch of times (without collecting the documents) to get a better feeling of the time (and the path) required to do it.

The Explorer medal may also be a painful, requiring to restart the playthrough. Be careful when navigating the elevator shaft section and avoid dying.
Act 1
Document 1: Border Security. Given at the beginning.
Documents 2-3: Sponsorship, Checking In. In the very start before leaving the apartment.
Objects 1-6: Sleeping Pills, Classical Engineering, Certificate of Achievement, Wrist Bands, Hand Grips, Roy the Donkey. In the very start before leaving the apartment.
Medal 1: Introspective. Find all the objects in Kate's apartment.
Medal 2: Systems Engineer. After escaping the elevator, examine the malfunctioning door. Just walk to a red-lighted door with a sign "To Security Control", at the end of the corridor behind you.
Document 4: From the works of Thomas Malthus / Page 13 / Bloated Existence. After the next cutscene. It's on the wall to the left of the locked door to Central Station. Look at it through MAVI and collect the document.
Medal 3: Item. Find an object outside of Kate's apartment.
Object 7: Queen of the World Replica. On the desk of the next room, the one where Kate downloads the hacking module into MAVI.
Document 5: 10 Years of Progress at Sea / A Vision of the Future. Behind the desk where the previous object was found.
Document 6: Review. On the ground, behind the desk at the opposite side opposite of the last one.
Document 7: Briefing. On the ground, to the right of the computer console.
Medal 4: Acrobat. Explore the ceiling area in the Shuttle Plaza. Still in the same room. When you jump onto a girder, instead of following the girder to the objective to the right, climb onto the girder, then jump forward again, then move to the right while you are hanging, then jump onto the second girder.
Object 8: Queen of the World Anniversary. On the second girder (where you got the previous medal). Just follow it to the right.
Medal 5: Panther. Dive under the closing door before the corridor floods on your way to security control. After going back to the stairs, you will go towards the same door as the "Systems Engineer" achievement. Right after passing it, the corridor will start flooding. Move/jump/run/swim as fast as you can to get to the door on the other side, before it closes down. You will need to swim down to go under it. If you need to press the button to reopen the door, you missed it. If you miss it: Load Game - Last checkpoint.
Object 9: Queen of the World Anniversary Globe. On top of the catwalk after the flooded corridor, just above the A2 and A3 signs. You will have to use the ladder to reach the catwalk.
Document 8: From the works of Thomas Malthus / Page 6 / The Truth. In the next room, walk towards the two vending machines, turn around and equip MAVI. The document is hidden in a corner to your right, near some chairs and a trash bin.
Document 9: A History of the Population Flood / 1798: Malthus Opens the Debate. After climbing the elevator shaft, at the dead body that is flashing green.
Medal 6: Systems Engineer 2. Inspect the encrypted door in security control before finding the frequency key. At the same room, look at the purple-lighted door with a sign "Stairwell Access".
Document 10: Suicide Note / Epitaph. Look above the dead body (from document 9) and you will find a hidden shaft. Climb it and you will find yet another green-flashing body, this time next to a pipe and near smoke and flames.
Document 11: Death or Glory: Malthusians versus Cornucopians. Go back to the elevator shaft where you came from (where an elevator had just fallen down). Climb up to the next floor, open the door, and find another green-flashing dead body.
Medal 7: Explorer. Find both hidden rooms in security control. Find the rooms described at document 10 and 11. This medal is quite glitchy, if you don't get it at first, you might need to restart your playthrough. Try to reach both rooms without dying.
Document 12: From the works of Thomas Malthus / Page 3 / The Simple Solution. Inside the room where the Chief Billingham is trapped. It is hidden at the wall with a bright blue-and-white logo. Equip MAVI to find it. You can get it even from outside, through the glass.
Document 13: Interrogation. After climbing up and right, you will enter a darker room right next to where the Chief Billingham is. Climb down the ladder and grab the document on the floor.
Object 10: Wanted: Mila Krass. On the desk of the same room.
Object 11: 10th Anniversary Commemorative Coin. In the room above the chief, on the desk near the door.
Medal 8: Ciphers. Crack the Malthusian frequency key to discover the first cipher in security control. Awarded after you acquire the first cipher in your MAVI.
Document 14: From the works of Thomas Malthus / Page 2 / The Coming Storm. After going downstairs you will find the Security Office room. Inside that room, open MAVI and look right. You can also acquire it through the glass, before entering that room.
Medal 9: Fire Fighter. Extinguish the flames in security control. After you use the computer console in the Security Office room, there will be a 60-second timer. Go back to the Chief Billingham as quickly as you can, then use MAVI to remotely unlock the door behind him. You must do it before the timer runs out.
Documents 15-18: Briefing / Morton, D-Sal Security / RE: FW: Morton, D-Sal Security / Chump, Briefing / SITREP. In the room with the Chief Billingham. Given when collecting the Chief Billingham character profile (the yellow object on a desk to the left).
Document 19: Fun Times / Honesty Corner. On the desk next to Chief Billingham.
Object 12: Commanders Cut. Between the computer console and the red-lighted door.
Medal 10: Collector. Find all the collectables in Act 1. Yes, this medal is unlocked before getting Document 20.
Medal 11: Nemesis. Discover who is behind the Malthusian attack. Just watch the entire broadcast that starts after the Chief Billingham is freed.
Document 20: From the works of Thomas Malthus / Page 12 / Harvest of the Unworthy. After swimming and going downstairs, at the Central Station Access room, point MAVI between the elevators and the phone booths.
Medal 12: Survivor. Complete Act 1.
Act 2 (misc. notes)
You finally get a gun in this act (actually, at the end of the previous one). If you want to get the Supersonic medal, only use sonic rounds from now on.

There are several combat-related medals that can be obtained in several locations. I'm listing them here.

Medal xx: Resourcefulness. Kill a Malthusian by exploding a barrel.
Medal xx: Bad Dream. Kill a Malthusian with an explosion while they are unconscious.
Medal xx: Humanitarian. Headshot a Malthusian while they are on fire or being electrocuted.
Medal xx: Vandal. Destroy 50 objects.
Medal xx: Mermaid. Spend 20 minutes underwater.
Medal xx: Mega combo. Score 7000+ points in one combo. After killing an enemy (or while killing it), shoot as many things as possible as fast as possible. People say it is easier on hard difficulty.
Medal xx: Combos. Get a total of 20 combos in one play through. Get as many combos as you can in Act 2 and Act 3 by shooting barrels, gas tanks, electric wires, glass windows and enemies. You can also get this in Challenge room after completing the game. Combos only count if an enemy is involved.
Act 2
Document 21: From the works of Thomas Malthus / Page 5 / Doctrine. Look at the right side of the giant TV. Use MAVI to collect it.
Documents 22-23: Re: Review, Re: Settling In. At the beginning of the act, go back instead of going forward. Go back to the flooded elevator shaft you came from. Swim down to the next door (mid-way through the shaft), open the door, swim forward and then right until the end of the corridor. Examine the glowing body. Although you can reach this location on the first act, wait until the second act to go there.
Document 24: From the works of Thomas Malthus / Page 15 / Will of God. Near the same glowing body, there is a tiny room to the left. Point MAVI there.
Medal 13: Deep Breath. Collect documents 22-24 in a single try, without using the air pocket. This medal is glitchy because the player has essentially only one chance to get it. If you don't get it, you have to try again in a new game. If you collected the hidden document in the wall and missed this medal, you probably need to wipe your game progress and start again (because hidden documents are preserved across playthroughs).
Medal 14: Justice. Kill your first Malthusian.
Document 25: From the works of Thomas Malthus / Page 19 / True Virtue. After climbing inside the vent, go left (the objective is to the right, ignore it). After getting out of the vent, use MAVI to find it on the wall, at the dead-end slightly to the right.
Medal 15: Infiltrate. Gain access to the Cycloconverters.
Medal 16: Heroism. Rescue Grimley in the Cycloconverters. In the CCTV feed, open the door before the Malthusian kills the second hostage.
Medal 17: Valour. Rescue both Billingham and Grimley.
Document 26: Cycloconverters Operational Overview. Right after the previous medal, there is a document on the desk.
Document 27: From the works of Thomas Malthus / Page 16 / Graves of Mankind. Use MAVI on the wall behind the desk from the previous document.
Medal 18: Espionage. Use CCTV to spot the Malthusian in the Cycloconverters B control room. You must be in the Cycloconverters B room (i.e. you must have crossed the door) in order to get this medal. Use use MAVI on the CCTV camera right in front of you and cycle the feeds.
Document 28: From the works of Thomas Malthus / Page 17 / Impure Temptations. There is a small staircase under the CCTV you used in the previous medal, leading to a dead-end with some green lockers. Use MAVI while looking at the green lockers to find the hidden document.
Document 29: From the works of Thomas Malthus / Page 18 / Unworthy Masses. Use MAVI on the wall to the left of the computer console used to disable/destroy the Cycloconverters.
Medal 19: Fighter. Destroy the Cycloconverters.
Document 30: Anxiety. After turning a valve and coming back to Cycloconverter A room, there is a small staircase leading to a dead-end with blue lockers. The document is on the floor.
Document 31: From the works of Thomas Malthus / Page 27 / Ultimatum. After getting the previous document, instead of going towards the objective, enter the door to your left (the same door you opened through the CCTV camera in medal 16). To your right there will be a seemingly empty "Security Office" room. Enter it, turn around and use MAVI on top of the door.
Object 13: Anxiety No More. On the ground, where there is a flame that goes on and off.
Medal 20: Fire Fighter 2. Use CCTV to remotely open the door and extinguish the flames in the mainenance corridor after the Cycloconverters.
Document 32: From the works of Thomas Malthus / Page 1 / The Sum of all Men. To the right of where you get the "Unit A24 - Cycloconverters/VFD Access" cypher. Climb out of the water using the ladder, walk right and use MAVI on the wall.
Document 33: From the works of Thomas Malthus / Page 22 / The Violation. On the left wall of the tiny flooded room behind "Engine Room Access" door. This is before you climb into the vents.
Act 2 (cont.)
Document 34: From the works of Thomas Malthus / Page 9 / Balance of Power. Once inside the vent, there is path to the left that is blocked by a blue grid. Use MAVI on the floor.
Document 35: Resistance. Right after picking up Gel Rounds and getting out of the vent, examine the dead body.
Medal 21: Stealth. Kill the first 3 Malthusians in the first VFD facility without being detected. You should have 4 gel rounds (two from the vent, plust two from the dead body). Wait until the first enemy comes closer, then quickly fire the gel round at him and explode it. A second enemy will investigate the explosion, then you just repeat the same strategy. After two are down, Scoot will tell you to check the CCTV. Walk slowly and carefully to the third enemy, and repeat the same strategy. Scoot will compliment you and you will receive the medal. If you don't get it, reload from the last checkpoint. If you still don't get it, maybe it is glitched. After receiving this medal, reload from the last checkpoint to continue working towards Supersonic medal.
Document 36: VFD Facility Overview. In the small Maintenance room (after killing the 3 Malthusians), on the floor.
Document 37: From the works of Thomas Malthus / Page 7 / Ravaged Earth. In the same room, use MAVI to look at the bottom-right corner of the TV.
Document 38: From the works of Thomas Malthus / Page 10 / Purer State. After hacking the console and leaving the small room, a door with a sign "Maintenance Access" breaks, flooding the area. Swim towards that door and use MAVI to find the hidden document in the small room that caused the flood. Alternatively, wait until you drain the area and go there on foot, instead of swimming.
Document 39: From the works of Thomas Malthus / Page 20 / The Cleanse. Use MAVI right after climbing the red ladder next to the big door labeled "Loading & Storage Depot".
Document 40: Miracle. On the desk of the small room just above the red ladder mentioned above.
Document 41: From the works of Thomas Malthus / Page 14 / Higher Purpose. Use MAVI on the wall to the right of the computer console of the room after the vent (the room with several hanging electric wires).
Object 14: The Prophecy of Malthus. Underwater, next to the fence corner that is close to the computer console and close to the underwater stairs (in the same room as the previous document).
Medal 22: Battle Joined. Destroy the VFDs to take the fight to the Malthusians.
Medal 23: Breach. Shoot the breach in VFD 2 to flood the room and escape.
Document 42: From the works of Thomas Malthus / Page 8 / The Cause. In the next room (the one with several flames), get into the vent by climbing on the vending machine. Once inside the vent, use MAVI on the floor to the right.
Medal 24: Draining. Drain the water from the flooded loading bay area.
Document 43: From the works of Thomas Malthus / Page 11 / Damnation. After opening 2 underwater valves, use MAVI in the underwater middle room.
Document 44: From the works of Thomas Malthus / Page 28 / Assisting the Convulsions of Nature. After climbing yourself out of the previous room, you come to a staircase. Swim down to the bottom of the staircase and use MAVI on the wall.
Document 45: From the works of Thomas Malthus / Page 4 / New World Order. After hacking the console on the wall, enter the Control Room, turn around and use MAVI on the wall.
Medal 25: Maelstrom. Use CCTV to flood the first loading bay. In the big room with all the containers, use CCTV feeds to find and open a door to "Stairwell Access".
Document 46: Launching Mankind's Great Hope. After you remotely open a door to put out the fire in the gap below a trapped officer, swim down into that door. There is a document on top of a yellow box.
Medal 26: Collector 2. Find all the collectables in Act 2.
Document 47: From the works of Thomas Malthus / Page 23 / Eternity. After you use barrels to lift a container, you will find a glass wall (to the right) in the next corridor. Shoot it and use MAVI on the wall (next to the "Exit 1A" sign).
Document 48: From the works of Thomas Malthus / Page 29 / Fatal Urges. After swimming through some corridors, you get to some stairs. At the very top of the stairs, use MAVI on the wall, near the door with a sign "Mess Hall".
Act 3
Object xx: Purus Suppressant Pills. Right at the beginning of the act. Does not count towards the total object count.
Document 49: From the works of Thomas Malthus / Page 21 / Futility. At the moving bridge area, use MAVI on the wall behind the stack of two containers, near the A1 sign.
Medal 27: Flood. Flood the corridors after leaving the loading bays. At the moving bridge room (now flooded), use a CCTV to open the door to the first corridor. This will leave the door open and will fill the corridor with water. On the next corridor, shoot the two glass walls. The next room has electric wires, use the CCTV there to open the next door, which will help flooding the next corridor. In the next corridor, also shoot the three glass walls (to small dead-end rooms).
Document 50: From the works of Thomas Malthus / Page 25 / From the Flames. At the corridor after the room with electric wires, there is a small dead-end room behind a glass wall with flames on top of the water. Use MAVI on the large machine in that room. If you look at the map, the corridor has an upside-down "T" shape, and the room with the hidden document is to the left, near a CCTV camera.
Document 51: Anniversary Preview. Still at the same upside-down "T" corridor, go to the room with two sets of stairs (when looking at the map, it is to the right). There is a document lying on the blue machine next to the exit door. It is very hard to see due to the camera. Try positioning yourself slightly below the water surface, in front of the sign "Thermal Transfer" above the exit door, near a hanging lamp. Then look to your side, to the large square-ish blue machine.
Object xx: Purus Suppressant Pills. After hacking two computer consoles, after passing a corridor, there is a dead body across the water. Does not count towards the total object count.
Medal 28: Destination. Reach the Shuttle Plaza. This medal is gained right after the elevator cutscene.
Object 15: Queen of the World Anniversary Flag. On the second bench to the right, after the elevator cutscene.
Object xx: Purus Suppressant Pills.
Document 52: From the works of Thomas Malthus / Page 30 / Moral Dignity. After the anniversary flag (object 15), climb some stairs, then go right and down. You will find a dead-end with elevators. Use MAVI to look behind one of the MediaShack advertisements.
Medal 29: Gravity. Launch 2 Malthusians over the side of the Shuttle Plaza. Either use explosives, or just kill the enemies. Then, while they are on the ground, use charged Sonic Rounds near the bodies to nudge them towards the edge. You must get two Malthusians falling off the edge at the same time in order to gain this medal.
Object 16: Official Anniversary Big Boy Hand. After getting Moral Dignity (document 52), go back to the main area and go down on the next staircase. The object will be on a bench.
Object 17: Official Anniversary Big Boy Hat. It is behind the "Eat:Stop" store that is closest to the stairs to the previous object. It is close to a line of fire.
Medal 30: All Items. Collect all the items in the game.
Document 53: Satisfaction. Go down at the next staircase (just before going up a ramp formed by advertisement signs). It is on a bench near the elevators.
Medal 31: Climber. Climb to the Shuttle Station from the Plaza.
Document 54: From the works of Thomas Malthus / Page 24 / Scales of Injustice. After the cutscene of the villain entering the elevator, use MAVI on the wall to the left of that elevator.
Document 55: From the works of Thomas Malthus / Page 26 / Duty. In the room after gaining the water powers, entering the elevator, and then leaving the elevator. Dive into the water to the left. Use MAVI on the base of the middle mechanical arm.
Medal 32: Collector 3. Find all the collectables in Act 3.
Medal 33: Geek. Complete the game without failing a hack. If needed, press the button to disengage the hack. Aborted hacks don't count as failure.
Medal xx: Supersonic. Complete the game using sonic rounds only. The wave power is also allowed (and required for the final boss). This medal is awarded upon completion, but it is not listed in the in-game database and does not count for the "Collect all the medals" achievement. It is likely a leftover from the earlier Xbox version.
Challenge Medals
Medal xx: Hydro Power. Complete the first 2 waves in the Challenge Room without firing your gun. Just use the wave ability.
Medal xx: Poseidon's Daughter. Complete the Challenge Room without firing your gun. Just use the wave ability.
Total

17 Objects.
43 Medals (only 42 counted in-game).
55 Documents (30 pages hidden on walls, 25 normal documents).
Credits
This guide was originally based on this guide for the Xbox version[www.xboxachievements.com], but was later reformatted and almost completely rewritten. Since there are differences between the Xbox version and the Prophecy version (Steam and PS3), the original Xbox guide was not accurate for the Steam version.

How to get the combos medal? Its my last one and im trying to do it in challenge room.Should i do it without dying here and get 20x combo in a single run to wave 5?

Thanks for the guide.

I was able to get Medal 21: Stealth using only sonic rounds though its tricky. For some reason when in the CCTV and backed into one corner the guards rarely find you.

In order to get Medal 4: Acrobat I had to set the game to 59Hz, turn on vsync, etc... had the best luck jumping when the fog turns red for some reason though. First time it worked great then I had to try like 25 times to get both jumps.

Medal 29: Gravity - the trick with the sonic rounds worked great my first try, then the second time through it was next to impossible... ended up pushing them back towards the start and over the corner, not even simultaneously, (close) and that worked. I think the bodies land on the pipes and get stuck sometimes?

One error: It says:
"Medal 13: Deep Breath. Collect documents 21-23"
This should really read documents "22-24"

Thanks for the guide, but I have to point out that the Gravity medal does not require you to launch two Malthusians at the same time. You don't even have to launch two living Malthusians, you can use sonic rounds to nudge two corpses over the side, under the railing, and it will count.

Thanks for the guide, especially for the heads-up about Deep Breath. It should be emphasized that the Supersonic medal (only use Sonic rounds) is NOT required for the achievement. I know that you mentioned it, but it should probably be highlighted in the introduction, not hidden in the wall of text. I spent ages trying to kill the boss without Energy rounds before re-reading the guide and finding out about Supersonic :P
