Introduction
So some people have been wondering how to unlock these two missions. After beating mission 44, instead of getting mission 45/46, the game skips over to 47. Missions 45 and 46 are the endings to the game. Mission 45 is the ending that ends Quiet's storyline, while mission 46 is the "true" ending with the biggest twist in the game. This guide will tell you how to unlock these two missions but there might be some spoilers included
How to Unlock Mission 45
To get mission 45, you should make sure that you meet the following requirements:
  • You must have spared Quiet at the end of mission 11, and then recruit her as a buddy. If you killed Quiet at the end of the mission, then I believe you can still get her if you replay the mission and spare her instead. To recruit Quiet, you need to do the yellow side op where you visit her cage. I had this side op come up about 5 missions after finishing mission 11
  • Raise your bond with Quiet to the maximum. You can raise your bond with her by taking her on missions, and you can check her bond level at the bottom left of the mission results
  • See the cutscene where the scientist, Huey Emmerich, is exiled from Mother Base. For me, this cutscene wasn't triggered right after mission 44. But, after doing mission 47 it did, so you may need to do a story mission to get this cutscene to trigger
After meeting these requirements, Quiet will leave your team and you will get the yellow side op 150, which will lead into mission 45. Beating the mission will give you Quiet's ending and the "Disappearance" achievement

WARNING: Quiet will leave your team after meeting these requirements, and you will not be able to get her back. The only way to keep her is if you have the butterfly on your Mother Base emblem. It doesn't matter where the symbol is or how big it is, as long you have it on the emblem, she won't leave you. Keep this in mind if you want her as a buddy

UPDATE AS OF NOV 10, 2015: With the most recent patch, you can get Quiet back if you replay Mission 11 7 times. Good luck!

How to Unlock Mission 46
Mission 46 is a little more difficult to unlock, which makes sense, since it is the last story mission in the game. To get mission 46, you need to meet these requirements:
  • Complete missions 1-44. Missions 45 and 47-50 are not required to unlock mission 46, and if anyone says you need to, then that is false. I got mission 46 without doing 45, 48, 49 or 50. Some people have claimed that you do not need to do all of the chapter 2 missions (31-44), but if you don't have mission 46 yet, make sure you start completing these chapter 2 missions just in case
  • Make sure you beat every yellow side op, excluding side op 150
  • See the scientist get exiled from Mother Base. This is the same custcene needed to trigger side op 150
  • Make sure you have Mother Base upgraded to a certain point. Some people will say you need it to be fully upgraded. This is also false. I got mission 46 with my command platform at 4, two other platforms at 3, and every other one was at two. As long as you've been upgrading your base platforms and developing items throughout the game, you should be in good shape. If you still don't have mission 46, then grind and keep upgrading your platforms until the mission appears
  • You MAY have to listen to all of the yellow cassette tapes. You don't need to listen to it all, just play each of them so that the dot on the side of them is gone. Some people said that they were able to get the mission after listening to the tapes, while others said it is unncessary. If you did everything else and don't have the mission, then try listening to these types
After meeting these requirements, mission 46 should appear on your iDroid. Beating this mission will give you the true ending, some credits to watch, new cassette tapes, and the "Truth" achievement
Conclusion
Hopefully, after reading this guide, you will have Mission 45 and 46 playable. If any of the content in this guide is false or unnecessary, then please leave a comment. I will continue to edit this guide to make sure it is as accurate and helpful to the reader as it can be

I did see Huey getting exiled. Bond with Quiet is lower than 100. I did complete the story missions. Did not complete all repetition messions.
I just listenned to all the yellow tapes, came back to my ACC, and missions 45 & 46 unlocked.

You don't need to do missions 1-44 to get mission 46. I just unlocked it, and as you can see I didn't do some of the missions. http://i.imgur.com/3jHgkL6.jpg

Other than the screenshot, I also didn't do mission 34 and 37.
