Introduction

Hello and welcome to this text and video guide on the achievements in this wonderful mod of Portal 2. Now this will contain instructions on how and where to get the achievements as well as a listing of the story related achievements so you know when to expect them. If you don't like spoilers at all then I would suggest skipping over them and heading straight down towards the "secret" achievements. The total time of completion to get all of the achievements is around 5-7 hours, maybe less if you can speedrun all of the puzzles.

If you are needing help I would suggest checking out the guide made by quaternary linked below where he/she shows through video guides how to complete each puzzle.
Solutions guide

Please note that all the videos below do in fact belong to me as they are also attached to my profile in my content as well. Please do not claim them as yours, give credit where credit is due. Do not redistribute this guide without either my permission or without giving credit again where credit is due. I found several copied and pasted solutions from one guide that I found on several different sites, I do not want to find the information I post here plagurized (if you must at least write it in your own words).
Story Achievements
Warning: Achievements listed below are semi-spoilery.

Welcome to Aperture
This achievement will unlock with normal progression after you first get off the train after a cutscene in chapter 1.

Long-Term Relaxation
This achievement will unlock with normal progression after you wake up in chapter 2.

Voices from Above
This achievement will unlock with normal progression during the Ramps track's (sp_a2_ramp) second part where you'll hear AEGIS for the first time.

Firefighting 101
This achievement will unlock through normal progression after you put out your first fire.

Back on Track
This achievement will unlock with normal progression in chapter 3 after you enter the elevator.

Deja-Vu
This achievement will unlock with normal progression towards the end of chapter 3 in the room that requires you to repeat the test 3 times. Do it and it'll unlock.

Organic Complications
This achievement will unlock with normal progression after you enter the room where Virgil tries to "hack" AEGIS. You will press a button, the achievement will unlock after. Alternatively, you can just let Virgil continue to "hack" the terminal and it wiill eventually unlock, the trade-off is time saved and continuing on, or time used to listen to more dialogue. The choice is up to you.

Back off Track
This achievement will unlock with normal progression after you either complete and escape the broken testing track or just skip it all together.

Welcome to my Domain
This achievement will unlock with normal progression when you enter AEGIS' chamber after going through all the prep work inside of Core Access.

System Shutdown
This achievement will unlock when you defeat the final puzzle and AEGIS as well. You've completed the story. NOTE: You will have to complete the "advanced" (aka original) puzzle track to unlock this.
"Secret" Achievements
Persistence
This achievement is only unlocked inside of the first chapter immediately after you get off the train until you get to the elevator. All of these doors are of the "locked" variety with the symbol you see in the achievement and make a noise when you press "E" on them. In the first area with the train there are 5 doors. Two on one side of the train platform and three on the other. Proceed until you reach the Aperture building and there in front of you is another door behind the receptionist desk. Go down the stairs until you see two sets of automatic doors, go immediately to the right side and turn 180 degrees and there will be another. Proceed down to the elevator area and on your right side there will be three or four more. Once in the elevator area there will be one BEHIND the elevators next to what look like hot water heaters. If this text wasn't clear enough please use the video below. I have also included a screenshot (not from the correct area, but the correct door type) of what kind of door to look for.

Testing the Waters
This achievement is unlocked after the elevator ride in chapter 1 where the relaxation center is. To the right there will be a body of water. Jump in it and the achievement will unlock. It has been reported by some that you can also unlock this achievement in chapter 2 after waking up and making your way out of the center. If further help is needed, consult the video guide below.

Curious
This achievement requires you to find all 5 all new Vitrified chambers, all of which are found in chapter 2's chamber "Past Power." On all of these press "E" on the little intercom on the side of the door. The first one is after you solve the first puzzle and have to walk on the repulsion gel pipe until you reach the other side, continue to walk until the end and DON'T ENTER THE ROOM, but at the end of the platform is your first chamber. The second and third rooms are after you solve the test. You'll enter the repulsion gel pumping station, before continuing go down in where the gel is dripping and there will be two more doors down here. The fourth will be on your way into the final area and in a hallway. The last one is in the final area of Past Power where you also have to do "You Shouldn't be Here" as well. When you emerge head down one level and to your left. The chamber will be at the end of this walkway. If more help is needed, please watch the video below.

You Shouldn't be Here
When you enter the final area of "Past Power" you'll have to make your way to the end. Follow the normal route until you make the jump. Now instead of going forward, stay on top of the testing room and shoot a portal off to your left where there is a tipped over panel. Go back to where you used a portal to jump the gap and place a portal on that surface. Make your way along the catwalks until it ends, jump down onto another catwalk and follow this catwalk all the way to the end. Once you reach the end there will be a Vitrifiied room that hasn't been sealed. Press "E" on the door and the wheel will turn before falling off, the achievement will unlock right after that. If more help is needed see the video below.

Beyond your Range of Hearing
This achievement is unlocked by repeatedly pressing "E" on Virgil the first time you meet him BEFORE turning on the power at the beginning of chapter 3. Clicking also seems to work (though in the video I did both). Some people have stated on some forums that this has unlocked in the final section of the game. If further help is needed see the video below.

Electrocution
This achievement can be gotten at several different places during the game, the earliest is in chapter 3 when you meet Virgil and have to turn the power back on. Jump in the water after turning the power back on, either you will unlock the achievement right away after dying or during the loading screen. See the video below if you are still having issues.
"Secret" Achievements Pt. 2
Under the Stairs
This achievement is completed inside the first testing chamber in Chapter 3. After you get both of the cubes needed to proceed in testing chamber on the side that you started on there will be stairs that rise up. If you're looking straight at the exit door look right and under the stairs, but at eye level. You will see a portal-able wall, shoot one portal there and go outside the blue barrier and shoot the other there. Pass through and then duck under the passage, go down the short corridor and hang a right. Entering this room with the artwork in the screenshots below will automatically unlock the achievement. If more help is needed see the video below.
EDIT: It was presented to me much later after the "Story Mode" update was released that you may have to run the Advanced difficulty in order for this to pop.

Ignorant
This achievement requires you to walk into the "death elevator" or "trap elevator" at the end of the official test of chapter 3 "Transition". Just walk into it and Virgil will tell you that it is in fact a trap, you need to stay inside it until it kills you; you cannot walk in and just step back out as that will not count. If you are still having issues with this consult the video below.

Forever Alone
This achievement is available in the chapter 4 map "Funnel Over Goo". After you solve the first test walk through the emacipation grid but before you enter the next testing track go down into the garbage and you'll see a Companion Cube. Crouch down and walk over to it. Press "E" and it'll disintegrade but you'll unlock the achievement. If further help is needed watch the video below.

Single Rainbow
This achievement is available during the chapter 4 test track "Two of a Kind", right after the "Funnel Over Goo" track. From immediately beginning the track drop the first cube and make your way to the button across the goo. Shoot a portal to where you started and place the cube on that button. Now with the funnel active place a portal where it currently ends and where you got the first cube. Ride the funnel to where the second cube dropper is. Step out. There will be a panel slightly off the wall, shoot a portal behind it and create another to get to the portal you just shot. Now walk to the end of the catwalk. Once the conversation's complete the achievement will unlock. If more help is required watch the video below.

Burned in Goo
This achievement requires you to burn yourself while in goo. Now this from what my viewers on YouTube have told me this is sort of glitchy. Now while inside of the broken testing chamber inside of the final portion of the Factory (a4_factory) in chapter 4 while you are attempting to either solve the puzzle or just skip it, go over to the laser beam. Now stand in the goo and just step into the laser beam until you hear a sizzle sound effect. In my personal experience all you had to do was walk into it. If more help is needed on this tricky achievement see the video below.

Determined
This achievement requires you to solve the broken test chamber in the Factory puzzle set in chapter 4 (a4_factory). This is an easy puzzle that shouldn't take you more than a minute or two. See the video below for more details.

Crushed
This achievement requires you to die while being crushed. Now this is only available to be done while inside of the Factory in chapter 4 (a4_factory). There will be a testing track where they are testing whether cubes can press buttons. Stand next to the door while it is closed, now when the cube presses the button to open it stand inside of the doorway. Once the cube depresses the door should close on you, killing you, unlocking the achievement. If further help is needed see the video below.

In the Vents
During the part of the Factory (a4_factory) in chapter 4 you will pass a turret shooting cubes. When you enter the office area you will pass through it. Now once you enter turn right and look inside the first door. On the floor there will be a portal-able surface, it may take you a few tries but place one there. Now go back out and turn right and enter the second door from the beginning. Shoot a portal on the ceiling. Enter the one in the first office and enter the door. Once through the door you'll see a vent, crouch down and enter. Once you pass through it you'll unlock the achievement. If further help is needed see the video below.

Into Darkness
This achievement is unlocked inside of chapter 4's Factory puzzle track (a4_factory). Once you get to AEGIS' core room you'll enter a dark area. Proceed until Virgil tells you to watch out for the hole in the floor. Ignore him. Jump down it and the achievement will unlock. If further help is needed see the video below.

2056
This achievement is unlocked inside of chapter 5 at the beginning during the Core Access map (aka Infiltration). Enter the labs and you'll see "2056" printed on the wall. Inside of that area you'll need to shoot a portal inside a crack in the wall and shoot a portal on the wall behind you. Enter and approach the graffiti that says "2056" and the achievement will unlock. If further help is needed see the video below.

Shot
This achievement is VERY misleading. This achievement is unlocked by getting killed by a turret, HOWEVER it has to be specific kinds of a turret as there are turrets aplenty through the game, however the one you need is inside of chapter 5 while doing the objectives to shut down the defenses of AEGIS. After you get the power back on and take care of the security you'll enter the turret production line. After you remove the template turret and go through the unlocked door you'll come to the "live turret testing range". Stand in front of these turrets and die, between the time you die and respawn you'll unlock the achievement. If it doesn't unlock right away retry again as there is an autosave by this door. If more help is needed see the video below.
Story Mode Update
Story Shutdown
This is an easy achievement, assuming you've completed the game once before. All you need to do if you have completed the game once before is load up Chapter 5 and play it through. Once you reach the cutscene after defeating AEGIS it will unlock. If you have not played this before, story mode is a lot easier than it was when the game was released, however it is just as enjoyable.

I did Under The Stairs in Story Mode.
Notably difference of System Shutdown in Advanced Mode is the placement of the hostile turret in the Aegis fight. If you don't destroy them and place them facing the wall, they don't respawn. The timer seems to be 1 minute less.

System Shutdown is literally the only one I needed advanced for, got all the rest in Story. One notable difference was "under the stairs" didn't have two cubes in Story, it just had one, but it still worked.
