Specifically it seems when it rains, the rich soil from it turns back into normal soil. This could also be an interaction with the Gulden Biome, but I think its also the rain mechanic from Nature's Pretty Sweet.
Not the end of the world, as I have also found out Mineralize makes a far superior moisture pump!
Don't really know how smart medicine works but compositable loadouts allows you to select any items and the pawns will go stock up on what you've set them to
