Level Related Achievements
You will likely finish the main campaign at ~level 47.
You will need to farm maps from fame contracts or Fazeer's Dun'djinn to reach max level of 60.
Fazeer's Dun'djinn has 251 levels of challenge. The first challenge starts at lvl 46 and goes up by one for each challenge.
Challenges will quickly outlevel your character. You can run older challenges to do easier levels, otherwise you can take a lot of spike damage if you go into challenges that outstrip your level by a 3+ levels even if you've got good defences.
Maps from contracts spawn at your current level so can be used to level up as well or saved until level cap.

First Class Hero

Reach level 60 with any of the classes except for Cursed Captain.

High Class Railmaster

Reach level 60 with the Railmaster.

High Class Forged

Reach level 60 with the Forged.

High Class Sharpshooter

Reach level 60 with the Sharpshooter.

High Class Dusk Mage

Reach level 60 with the Dusk Mage.

High Class Hero

Reach level 60 with Duskmage, Forged, Railmaster and Sharpshooter.

High Class Cursed Captain

Reach level 60 with the Cursed Captain.
Kill Related Achievements
Zombie Containment

Kill 500 Zombies.
Zombies can be found in the first few zones in act 1. Should get this just by playing normally.

Never Netherim

Defeat 500 Netherim
Kill Netherim enemies. They can be found all over in story dungeons in act 1. Should get this just by playing normally.

Exterminator

Defeat 500 Hyvid
Kill hyvid in Act 2. Pretty much every enemy in Act 2 is a hyvid so impossible to miss.

Gobsmacked

Defeat 1,986 goblins using Entropy as the finishing blow.
Entropy is the final level 20 skill in the Dusk Mage's Dark skill tree. You can find goblins all over Act 1, sprinkled in a few places in Act 2 / 3 and in random dungeons (pick ones with goblin icon). Just level up a Dusk Mage till you get Entropy and kill goblins with it. Entropy is pretty much a core skill for me in Dusk Mage and this is fairly easy to get.

Terminator

Defeat 800 Automations using Shotgonnne Blast as finishing blow.
Shotgonnne Blast is a level 15 skill in the Forged's Barrage skill tree. You can find automations in a side dungeons in first couple of acts. Act 3 has a number of zones with automations as well. In random dungeons, select the levels with the icon of a machine.

Bad to the Bone

Defeat 1,982 skeletons using Scout's Bones as the finishing blow.
Scout's Bones is a level 1 skill in the Sharpshooter's Adventurer skill tree. Witching Grounds in Act 1 is a good spot to easily grind skeletons. Otherwise, pick up kills as you clear out infinite dungeon challenges.

Thunderstruck

Defeat 1,990 Voltura using Shocking Rounds as the finishing blow.
This is easily the most frustrating of the class kill achievements to do imo. Really slow and annoying to get the kills. Haul a max level Railmaster to the Grand Hall in Act 3. Run up to pack of Voltura, trigger Shocking Rounds, afk while train kills enemy. Wait for Shocking Rounds to cool down then attack next group. Rinse and repeat. Reload when level is empty. Tedious and boring.

Monster Hunter

Defeat 1 million monsters.
Eff whoever made this achievement lol as it's a hell of a grind. I think I heard it was bugged earlier at release which would explain how so many apparently have completed this achievement. I got about half the kills playing numerous characters for challenge to max level and through swaths of the end game dungeon. Then I grinded out the second half in a fraction of the time with an optimal, efficient approach.

For optimal grinding, get a max level (I used a Dusk Mage, it worked pretty well) on Practice (the easiest) mode and take the Electrode relic. Electrode is taking for the Lightning Storm ability as you can just turn it on and it will randomly strike enemies around you speeding up clear speed.

Finish the campaign on the character then reach 60 in the end game dungeon. Levels 60-63 have the 'Dividing' effect; whenever you kill an enemy they divide into two new enemies. More enemies = faster kills. Do goblin levels as goblins are plentiful but weak letting you kills fast. Specifically you want to look for goblin levels that have the effect that removes loot drop while having infinite respawn. This will let you grind that one level for kills until you want to puke. Tedious and boring and will still take a long wile to grind kills but you will get the kills required much faster than playing normally.
Gear and Set Related Achievements
Weapons of Choice

Pick up each type of weapon: Bow, Chest Cannon, Digitus, Focus, Greatweapon, Hammer, Mace, Pistol, Rifle, Staff, Sword.
Play each character a few levels, you should find everything you need.

Geared up

Pick up each type of armour.
Play each character a few levels and you should find everything you need.

Pampered Pet

Equip a pet collar and tag on your pet.
Play until you get far enough for pet gear to drop then equip it on your pet.

Elite Defence

Equip items with at least 1337 defence.
Shouldn't be hard to get, I got it near the end of the second Act. Just equip gear with +defence.

Legendary Loot

Have a legendary quality loot drop.
Should be one of your earliest achievements honestly, they tend to drop fairly often. Legendary items are special items that have fixed effects they always along with the usual random rolls. Some of these effects are great, some are meh. Lock the best ones in the Legendarium when you're ready to replace the Legendary item with an upgrade.

Legendary Collection

Collect 25 legendary items.
You will get this playing naturally over time as you work on the grindier achievemnts.

Set Achievements

You should find some set pieces during the campaign but will likely need to do some additional farming of the endless dungeon to complete each set. Class exclusive sets only drop when playing said class so you will need to grind for class sets on each character.

The best way I've found to target farm for individual pieces of class sets you are missing is to spend your gold gambling for them. Unique drop chance is pretty low from gambler but you can target farm for a single piece this way and eventually get it. It's not like there's much else for you to use gold on at endgame besides Receptables.

For non-exclusive sets, they each zone/monster generally has a specific set they can drop but I believe you can get them via gambling as well.

Store non-exclusive set drops on a mule or something and once you have the six required pieces of a set drop them into your stash then equip on a char. Expect a lot of grind.

Woodbeast Garb

Equip the legendary Woodsbeast set.
Dropped by goblins in Act 1.

Skittering Garb

Equip the legendary Skittering set.
Dropped by spiders in Act 1 and Hyvid in Act 2.

Arc-Powered Garb

Equip the legendary Arc-Powered set.
Dropped by Voltura in Act 3.

Musketeer Garb

Equip the legendary Musketeer set.
The Sharpshooter's class set, grind with the class until you find all six pieces.

Celestial Garb

Equip the legendary Celestial set.
The Dusk Mage's class set, grind with the class until you find all six pieces.

Mountain King's Garb

Equip the legendary Mountain King set.
The Forged class set, grind with that class until you find all six pieces.

Cosmic Garb

Equip the legendary Cosmic set.
The Railmaster class set, it is kind of bugged in that there is no actual 'Cosmic' set in the game. The set appears to be mislabeled after the Winterweave set (which has no achievement for some reason). So grind with the class until you find the six pieces of 'Winterweave' that has buffs for Railmaster skills.

Legendary Set

Equip each legendary set: Arc-Powered, Celestial, Cosmic Mountain King, Musketeer, Skittering, Woodsbeast.
Collect and equip all sets in game except for Winterweave which has no related achievement and Commodore which was added post release.

Commodore's Garb

Equip the legendary Commodore's Garb set.
The Cursed Captain class set, grind with that class until you find all six pieces.
Miscellaneous Achievements
Break All the Things!

Destroy 2,010 breakable objects.
There's a number of objects you can destroy during combat. You'll likely destroy them just flinging AOEs and the like around and should get it naturally while playing through story.

Savant

Spend ten points in any skill.
When you level up you gain 1 (or two points on multiples of five) skill points you can use to unlock and upgrade your skills. Put 10 skill points into a single skill to unlock achievement.

Replenished

Pick up 500 health potions
Pick up health potions dropped by enemies, they will automatically be added to your inventory if you have space if you're close enough to them. You can have up to 20 potions on your at one time. If you do not have enough space, the potion will remain on the ground until you have space or instance despawns.

Superior Enchanting

Use the Enchanter's Altar to slot 25 Enchanting Slots.
The Enchanter's Altar will be unlocked for your fort near the end of Act 3. In the post campaign random dungeons, gear can randomly spawn with an Enchanting Slot. This gear can be enchanted with a buff from a list of available options you can acquire from the initial round of fame contracts and random drops from post campaign dungeons. Put enchantments on 25 pieces of gear.

Money Tree

Grow an Ancient Luck Tree in your fort.
Plant the luck tree seed in your fort once you unlock it. Sacrifice items to the tree until it maxes out and reaches +5% loot luck.

Fight for your Rites

Use 100 shrines.
Shrines are scattered around levels. Click on one to deplete it and gain a specific time-limited buff. Shrines are regenerated each time map is reset. You will get this playing normally.

Rolling Stone

Collect 500 stone.
Hit boulders in levels, then collect the dropped stone. Stone is used for your fort.

Heavy Metal

Collect 500 metal
Hit metal deposits in caves then collect the dropped metal. Metal is used for your fort.

Woodchuck

Collect 500 wood
Hit trees found in levels and then collect the dropped wood. Wood is used for your fort.

Millionaire

Collect 1,000,000 gold.
Collect drops and sell to vendor. Drops all sell for the same fixed price so this can take quite a while. Nowhere near as bad killing 1 million monsters but still quite a grind. Max gold cap is 500,000 so you will need to spend some gold on gold sinks. You can buy some fort decorations, buy receptacbles for 2,500 each or gamble for set gear that you're missing.

Lore Master

Find 25 lore objects.
