To fix lag...
Most lag issues arise from the game not correctly identifying the refresh rate your monitor is set to.
To fix this in 2 easy steps:

Get your monitors refresh rate.
On Windows 10/11, go to Start > Settings > System > Display > Advance Display. (Make sure you primary monitor is selected, and note the rate next to the resolution.)

Change the refresh rate in the games config file to match your screen.
In Steam, go to your library. Right Click the game in the list, then Manage > Browse Local Files. Open 'Config.ini' with a text editor like Notepad. Change the setting 'RefreshRate=Unknown' to your monitors rate (For example, 'RefreshRate=60.00Hz'), and save the file.

The game should now run smoothly with V-Sync enabled. Alternatively, you can just disable V-Sync, but the game is very choppy and has a lot of tearing without it enabled.

Credits to BloodNile for originally finding this solution.
