Legend
Beat the game on Legends Never Die.
Legends Never Die is actually fairly difficult, even if you're familiar with boomer shooters. It's probably not a good idea to start on this mode, the game even warns you that it's unfair. With all that said, it's manageable if you couple it with a 100% secrets/kills playthrough. The secrets contain more than enough items to make most encounters a breeze. Save some apocalypse runes for the final boss so that you can instakill the enemies that spawn throughout the fight.

Speedrun Achievements

Speed Demon
Finish a level faster than par time.


Sprinter
Finish Chapter 1 faster than par time.


Super Quick Guy
Finish Chapter 2 faster than par time.


Pheidippides
Finish Chapter 3 faster than par time.


Aside from Sprinter, I got all of these on my first playthrough. Chapter 1 par times seem less lenient than in Chapter 2 and 3, so just run through it with cheats enabled if you're struggling to get the achievement.

edit: Apparently using chapter select and completing the last level of a given chapter under par time nets you these achievements too. Thanks to Muckelchen for the find!

Kill Achievements

Master Of Giblets
Gib 100 enemies
You will get this naturally as you play the game.

Fists Of Doom
Punch 50 enemies to death.
I got this on my second playthrough without trying, some secrets contain berserk pickups and you can buy berserk potions from the secret merchant if you're still missing some kills for this. You can also probably just load up a level on easy and punch everything to death to get it.

Executioner
Behead 100 enemies.
You will get this by just playing the game.

Burn Baby, Burn
Kill 100 enemies with fire.
Got this on my second playthrough, fire is very useful in this game so you should get this naturally.

Ice Cube Maker
Freeze & Shatter 100 enemies.
I was missing around 30 kills for this after my 2nd playthrough, so I ran through a couple levels with cheats on, blasting enemies with the ice staff until I got it.

Poison Master
Kill 100 enemies with poison.
Same as above except I switched to poison on the staff.

Shield Basher
Bash and kill 50 enemies.
You have to kill 50 enemies with the shield bash. Easy if you remember to deliver the killing blow with your shield from time to time.

Corpse Creator
Kill all enemies in a level.
Just kill everyone, you'll get this when going for 100% anyway.

The Green Knight
Defeat the Green Knight
You'll have to kill this guy in the first secret level on Legends Never Die if you're going for 100%, which is actually kind of difficult. Make sure you haven't missed any secrets before fighting him, the items you find on the way will make the fight much easier.

The Questing Beast
Defeat The Questing Beast
You can find the Questing Beast in the second secret level. Again, on Legends Never Die this boss is pretty tough, I had to use most of my supplies to beat it. Clear the surrounding enemies with a grenade or two and throw everything you have at it.

Giant Spider Slayer
Kill the Giant Spider.
This boss is located in the third secret level. I didn't have any trouble beating it, you can use a summoning rune, hop on a nearby rock and watch your summon decimate the boss even on the highest difficulty level.

That's No Ordinary Rabbit
Kill the Killer Rabbit
Can be found in a secret area of E3L4, Castle Ruins. I chucked a holy hand grenade at it and killed it instantly.

Heads Up!
Kill an enemy with a severed head.
I got this by loading up the first level on easy difficulty and running through to the last combat encounter where the exit door is. Save before killing any enemies there, then decapitate someone. I found that trying to aim at a distant enemy with severed heads is too tricky. Instead, try to position a low health enemy right in front of the severed head, then kick it into him. The distance shouldn't matter and he will die, getting you the achievement. It's still quite random to pull off, so you might have to reload a couple times.

Arena Achievements

Forest Arena Champion
Win the Forest Arena.


Frozen Arena Champion
Win the Frozen Arena.


Fire Arena Champion
Win the Fire Arena.


Final Arena Champion
Win the Final Arena.


All straightforward, you get just enough supplies to win, try to save more powerful items/weapons for tough waves. I found the Forest Arena the most difficult - you don't get that many items and the boss is a giant two-headed wolf, it can decimate your health in seconds and chases you down relentlessly, it's easy to get stuck on something and die. The Final Arena can get tricky too, but the runes you get allow you to "skip" the most difficult waves.

Gauntlet Achievements

Kombat Champion
Test thy might
Earned by completing the Arthurian Kombat gauntlet. You have to kill every enemy type in the game, one after the other, armed with just an axe and a shield... but you can also use your fists, so wail on everything with your left fist and the axe at the same time.

Gauntlet Master
Survive The Gauntlet Of Rage
You only get berserk and strength potions, so you have to be fast and beat each section before berserk runs out. You get a berserk potion after each boss so use those to heal. Save your strength potions for the latter half of the gauntlet, too.

Horde Master
Kill 200 enemies in horde.
This is much easier on the second horde map, get the required amount of kills in the first map to unlock it then play until you reach 200 kills. You get a ton of items before the gauntlet starts and periodic drops throughout, shouldn't take more than a couple of tries.

Secret-related Achievements

Medieval Detective
Find all secrets in a level.
Self explanatory, you'll get this while going for 100%.

Artifact Hunter
Collect all relics.
Also impossible to miss while going for 100%.

Ancient Celtic Level
Find a secret level.
Same as above, the first secret level exit is in E1L3 Dark Woods.

Naughty List
Slay the magical sleigh rider.
There is a santa statue in one of the secrets in E2L1 Village. Destroy it and the achievement is yours.

100%
Kill all enemies and find all secrets in all three chapters.
100% kills isn't really a problem in this game, I haven't had any issues with disappearing or hard to find enemies. As for all secrets, I suggest doing this on your Legends Never Die playthrough because the secret items are invaluable on max difficulty. When in doubt, I used these videos by Skid's FPS Corner, all the secrets are well documented in them:

Misc Achievements

Magic Man
Use every rune.


One Man Army
Carry every weapon in the game.


Self explanatory, you should get these naturally by playing the game.

Final Measures
Destroy the skull of the Dark Master
You have to kill the final boss, then quickly destroy his "corpse" (the skull he leaves behind) before the game ends. It's easy to miss this achievement (the time window is only a few seconds), but thankfully you can load the level instantly with level select, kill the boss with cheats and try again.

Black Market
Buy an item from the secret dealer.
The secret dealer appears in New Game+ near the start of the first level of each episode. Buy anything from him and it'll net you the achievement.

Smashing Pumpkins
Smash a pumpkin on Halloween
Simple but time gated, you can wait until Halloween or change your system date to the 31st of October. Load up the first level, go to the secret merchant (over the hill to the right when you leave the starting house). There should be a pumpkin next to him, pick it up then use it from your inventory.

Mysterious Gem
What's this thing do anyway?
Open the Bestiary page in the pause menu. There is a purple gem right in the middle of the screen. Keep clicking on it until you get the achievement.

Money Bags
Collect 5,000 Gold
I got this achievement on my second playthrough right before the final boss. I still didn't have enough gold to buy some achievement items from the merchant though. If you find yourself in the same situation, there is a way to farm gold with level select. Warp to the final level of Episode 3. There is a cracked wall behind an armor pickup to the right of the boss door. This secret room contains 2 gold bars (200 gold). All you have to do is collect the gold bars, warp to the final level again, collect the gold, repeat until you have as much gold as you want. If you warp to another level from here you will keep the gold in your inventory.

Broken Shield Of The Ages
Break The Godly Shield Of The Ages
The Godly Shield Of The Ages is a shield sold by the secret merchant in NG+ for 1200 gold. You might not have it unlocked depending on how many quests you've completed, I suggest doing those before trying to get this achievement. Once you have the shield, just bash anything with it until it breaks. The durability might seem like it's not depleting, but it will wear down eventually.

A Link To Thy Past
Kill an enemy with the Wooden Sword
The Wooden Sword can be bought from the secret merchant in NG+ for 3000 gold. I didn't even have that much after my 100% secrets playthrough, but with the aforementioned gold farming method you should be able to buy it. Kill any enemy with the sword's projectiles and you'll get the achievement.

To make the trick to farm the 5k gold more clear:
the level warp you mentioned is using cheats, which you first have to enable in the main menu and then ingame open the cheat menu (you can look up and change the controls for that).
Also it seems you don't need to collect 5k on one profile, as I was only around 1k gold on that profile when I got it because I had already collected around 4k on my other profile.

Also one important info about the wooden sword as it took me like 10 minutes to find it after buying it: It's the only weapon that is on weapon slot 0, so it does not show up on your weapon inventory in the hud!

One more thing: You can easily get all the speedrun achievements by just using chapter select, selecting the last level of a chapter and finishing it under par time. Just play on easy and it's really not an issue.

I am still in my first run but I have a few notes:

-Regarding all the kill achievements: you can pretty much farm them at one spot. Just save before an area, then kill all enemies in that area the way you need to, then reload and do it again. The kills won't be "forgotten".
-You don't really need berserk to get fist kills, strength potion also works quite well.
-I am doing my first run on " Legends Never Die" and I don't think it's that hard. Just save before every encounter and go for 100% secrets and you are fine.
