This is the Shotgun Collar Trap, seen in SAW III.
Features a map, working trap and John Kramer himself, in the hospital bed

Instructions for the trap. Listen carefully, if you will. Do not get confused

Keep John Kramer (Jigsaw) alive. If you keep him alive for long enough, you will win the game. It's as easy as that..
However, you can kill him, and suffer the consquences.

Features a PLAY ME tape, that is not related to the trap.. But you can read it near the end for a expolsive surprise
