Rulers will always have helms/headgear on when they are commanding armies. Independently of the tier of their titles.

@Anubis don't take the scholar lifestyle perk in learning tree or the August perk in diplomacy tree. If you are a european. Some western cultures get that laurel wreath (leaf) when they take that perk. You can use the cheat mod called Divine intervention to remove those two perks and it will get rid of it.
