As far as I can tell only, a single value decides whether your save is dead or alive. For me, my current entry is
3E C2 AE 91 31 70 AA 54 35 DF DC BD BC 70 1E D8 57 ED 65 2D A4 16 57 04 11 8E AA CD 39 00.
When making a new save, only the first 3, last three before 00 and the value that is currently "65" changes. However, when I die and my saves become unavailable, the value "35" in 9th position changes to a "34". When I only change this value back to "35" and restart the game, I can play any save of the run that is supossed to be lost. My guess is that the values that change between saves only determine the time at which the last legitimate insane save was made and the values that dont change are either system specific or also part of the time values.
Im not sure if the 34-35 and the position of this value is the same for everyone. You may have to figure out at which position your "death counter" is and what value corresponds to "alive" and "dead".
After doing the first playthrough in 2022, I finally returned to get the second one done.
Ill share my findings that were inspired by a previous commenter
When you die, your saves dont actually get deleted.
What actually happens is that a token in the windows registry gets changed so the game knows internally that you died.
To be able to play from a "dead" insane save, you just have to change back the token:
1. open "registry editor", which is preinstalled with windows.
2. In the folders on the left, navigate to Computer\HKEY_CURRENT_USER\SOFTWARE\CD Projekt RED\The Witcher
3. There, you will find a list of entries in the format "name/type/data". The one governing the state of insane difficulty ist called "GameData"
4. Double-press the "GameData" entry and it will show you the complete hexcode.
5. find the value "34" and change it to "35"
6. restart your game and the insane saves should be available again
