Outlast - Note's & Documents (Video & Text Guide)

Welcome everyone, to the Outlast Collectible Guide, I’ll be showing you where to find all the notes and documents. Additionally, there will be a written guide available if you prefer to read through it.

This guide is designed to be as spoiler-free and jump-scare-free as possible.

Administration Blocks

Document: The Whistleblower

You obtain this automatically in the car.
(Story-related and cannot be missed.)

Note: Mount Massive Asylum

Go through the gates, pull out your camera, and focus on the building to get the note.

Document: Project Walrider Patient Status Report for William Hope

Once you’re inside, squeeze between the boxes. In the second door on the left (marked with blood on the wall), you’ll see a desk with the document on it.

Note: I’m Inside

When you see a man impaled on a spike, raise your camera to record the scene, and you’ll get the note.

Document: Warrant for Seizure

After waking up, head to the main desk. The document is by the keyboard on the right.

Document: Project Walrider Patient Status Report for Chris Walker

From the last location, go to the right and through the opening. Look toward the light, enter the nearby door, and you’ll find yourself in a file room. Turn right, then head to the far-right side. Turn left, and on the third shelf (from the bottom), you’ll find the document.

Note: Broken Men and Dead Television

From the last location, go through the door on the right and follow the corridor. Pass the person in the wheelchair and enter the open door on the left. Use your camera to view all three people sitting down, and you’ll get the note.

Note: The Witness

Starting from the main desk where you found Document 3, go to the back, and you’ll see an elevator. Turn right, follow the corridor, and head to the toilets on the left-hand side. Open the door with blood on the floor, use your camera, and focus on the walls marked “Witness” in blood to get the note.

Document: Project Walrider Profitability Report

From the last location, turn around and follow the corridor to the last door on the left. Go inside, and you’ll find the document on the desk at the back (it will flash blue, making it easy to spot).

Note: Big ♥♥♥♥♥♥♥ Guy

In the security room, wait for Chris Walker to break through the door. Record him with your camera to get the note.

Document: MKULTRA CIA Hypnotic Suicide

Exit the security room and turn right. Go toward the elevator and head down the stairs. After squeezing through the gap in the brick wall, turn right, and you’ll see the document on the table.

Prison Block

Make sure you read all notes and documents.

Note: Father Martin’s Cell

As soon as you wake up, use your camera to record the cell you are in.

Note: Necromantic

After leaving your cell, head down the stairs and take the 4th door on your left (this is the second-to-last door). Go through the gap in the cell, climb up, and record the guys in front of you.

Document: Father Martin, Finger Painter

Follow the pathway around and go through the sliding doors at the end. Go up the stairs until you see a guy saying, "Down the Drain." Turn left at the end, and you’ll find the document on the table.

Document: Purge Gate Maintenance Memo

When you see the naked guys talking again, climb out of the window. Shimmy along to your right, then climb through the broken window. Turn left, and you will see another broken window. Jump through it, turn to your right, and the document is on the table in front of you.

Note: Total Security

From the last location, jump back through the window. Go into the door on your left, where you will see Chris Walker killing someone (on the ground floor, to your left-hand side). Record Chris Walker as he kills the guy to get the note.

Document: Dr. Wernicke Death Certificate

Grab the key card from the dead guy at the end of the corridor. Before leaving, go back through the sliding doors and turn left (you will see a red arrow on the wall). Follow it until you come to a door. Use your card to unlock the door (SHOWER ROOM). Open the door, go through, and turn right. Follow the path until you see a broken window. Jump through it, turn right, and continue to the end. Climb back through the broken window, keeping to your right, until you reach a door. Open the door, turn right, go through the doorway, then turn left and right again. The document is on the table.

Document: Dr. Wernicke Obituary

In the area where you found the dead body (where Chris Walker was when you got Note 3), climb up the beds. Turn right and continue on. You’ll need to shimmy along the wall. Once you’ve done that, jump to the ledge, and the first cell door on your left will have the document on the desk.

Note: Walrider

Shortly after grabbing Document 4, go up the stairs. Shimmy along the wall again, then enter a cell and crawl through the hole. Follow the blood down the hole. When you drop down, look for a massive pool of blood on the floor (you can’t miss it). Record the room to get the note.

Sewer

Document 1/1 The Gospel of Sand

At the start of the sewers you will have to eventually crawl through the hole you will see something move across the screen ignore that and keep moving forward towards the light, turn left and go around untill you see blood on the floor on your left handside the document will be there.

Note 1/2 The Doctor Is Dead

When you see a flashlight and you will hear Father Martin the next door you enter you will see someone in front of you record him talking to you

Document 2/2 Still Bleeding

When you get into the main sewer section (you will know because its very very very dark)
once you jump down in the water for the second time you will need to walk up the stairs, Go around the corner to your right you will see a dead body hanging over the rail, from here drop down back into the water again you will see some light off to your left, ignore that and keep going forward you will see a walkway in front of you, jump and climb up here open the door and grab the document next to the body

Male Ward

Note A Feast for Flies

When you go through the bloody sewer water and you will see a guy run away from you, then go the only way you can go through the gap and you will see stairs on your right and a sign that says "Male Ward" ignore that and keep going forward you will see a door on your left record the dead body's

Note Organized Torture

As soon as you enter the male ward you will need to crawl through the vent, you will see a guy sitting on the chair head through the door and turn left and take the door on your left and record the words on the wall

Document Project Walrider Cost Report

when you go through another crawl space you will now be in a ward goto the doors in front of you and turn right when you get to the doors to find the document

Note Dr. Rick Trager

When you break free leave the first room you are in and take the next door out, you will see someone to your right on the bed there walk towards him and he will start talking now hide somewhere and record him untill you get the note

Document Request for Reassignment

When you Climb Through your second air vent (when running away from the doctor) you should drop down right in front of some toilets open the far right door in here to get your document

Note Death of Trager

when you kill the doctor climb through the hatch and record the doctors Body from behind you will get the note

Document Persecutorial Delusions

When you see Father Martin again he will say meet me outside
take a left and head into the locker room go straight and pick up and document next to the lockers

Notes Let it Burn

when you are in the burning cafetiere area you will walk right pass the guy so record him to get a note

Documents Psychopathological Proximity Stress Disorder Pamphlet

When the leave the burning cafetiere room take a left then a right in the dark area go the only way you can pick up the document on the table

Notes Fingers

When you have turned the water on to extinguish the fire in the cafetiere now head into the kitchen and take a left, go into the door and record te bowl of fingers

Court Yard

Document: MKULTRA Program Excerpt

before you head outside (the door will have an Exit above it) take the room to your right and pick the document up on the desk

Document: Our Own Personal Calvary

when go outside for the very first time go straight and keep going you should see some stairs of to your left eventually go up them pick up your document.

Note: The Thing in the Dark

once you find the key for the shed to open it go through the door and turn right, open the door and get your camera out ready for the note (KEEP RECORDING DONT LOOK AWAY) record the shadow the comes after you

Note: Don’t Drink the Water

When you see Chris Walker Again go through the crawl space you will see a big fountain full of blood, record it to get your note

Female Ward

Note: The Sound in the Machine

When you see farther martin again squeeze through the gap climb through the window and record the middle washing machine it will have a head inside it

Document: Walrider and Nightmares

From the last location of note 1 go through the door on your left then go into the door on your right now turn right and go straight all the way until you see some stairs look under the stairs for your document

Note: No Safe Place

From Document 1, now head back down all the way down the corridor until you see a guy behind the door turn left and you will see a wheelchair look left now and you will see an elevator record the dead guy in there to get your note

Document: Project Paperclip Excerpt

from Note 2 the door next to the elevator on the right and head down the stairs you will be in a dark room the document will be on the far left hand side corner on the desk

Document: You Promised Me a Rose Garden

After you have done the 3 fuses and grabbed the key, you will have to eventually shimmy along a wall you will also see a guy down there, jump across the gap, then you will enter a room and fall through the floor climb back up using the beds, take the door on your left, when you enter the next room the document is in the far left hand side of this room

Note: Lies

From Document 3 turn around and go forward until you see broken double doors, from there take a right then a left then another right into a room that says "LIE" use your camera to get the note

Return To The Administration block

Document: Billy’s Dreams

When you see Chris Walker again bashing through the door, you’ll notice a door that is slightly open. Look for a vent cover hanging from the vent. Climb into the air vent and take your first right when you can. You’ll see the document in another room, so drop down and grab it.

Document: The Spirit Breach

Push the body out of the way while in the vent. Drop down and take the first door on your left labeled "Library." In this room, the document will be in the top-right corner.

Note: The Wernicke Exit Interview

When you get into the theater, the projector will turn on. Just turn around and record the video playing on the screen with your camera. You must watch it until you receive the note.

Document: Necrotizing Fasciitis

From the location where you got Note 1, turn around and you’ll see a door at the back of the theater. Enter that door, then go through the next door as well. Run straight past the lockers and stairs, then enter the next room. The document will be there for you to grab.

Note: Static Prayer

After getting the key from the theater, head back to the door to unlock it and continue up the stairs. Go through the kitchen, and you’ll enter a large room where a guy is looking out the window. Take the door to your left in this room, turn left, and then take the first door on your right. You’ll see a TV and a guy. Record them with your camera to get the note.

Document: The Gospel of Judas

From the location where you got Note 2, leave the room and take a right. Go through the gap in the wall, leave the door, and turn right. Take the third door on your right, then pick up the document in the room.

Underground Labs

Note: Under the Mountain

When you first enter the underground labs, record the desk in front of you to get your first note.

Document: The Pride of Wisdom

From Note 1, go behind the desk to pick up your first document.

Document: Variant Postmortem

From Document 1, continue down the corridor past a sign that says "A Block." Enter the double doors, then look for a second set of double doors to your left. The document will be inside.

Note: Morphogenic Formula

From Document 2, exit through the door and turn left. Take another set of double doors, passing two "A Block" signs. After the second sign, turn right and enter the door. You’ll find a board with information on it—record it to get your note.

Note: Death of the Soldier

This note unlocks automatically.

Document: Gods and Monsters

While running away from the Walrider, you’ll enter another decontamination door. Turn right, enter the double doors, and look for a desk near a body with a sign that says "C Block." The document is on the desk.

Note: Wernicke’s Machine

From Document 3, take the double doors on your left (while still facing the document). Wait for the massive doors to open, and record the big ball in front of you to get your note.

Document: Morphogenic Engine Chamber Precautions

From Note 4, proceed through the door and take the stairs down in front of you. You’ll find a control panel with the document on it.

Note: Billy Hope

Next to Document 4, record Billy (aka The Walrider) inside the dome to get your note.

Note: Life Support

From Note 5, look to your right while still facing Billy. You’ll see a set of double doors in the distance. Go through them, then take another set of double doors and record what's in front of you to get this note.

Document: The Modern Prometheus

From Note 6, continue up the stairs in the same room. Take the next door on your left to find the document.

Document: Permission to Proceed

After turning the valve off, you’ll need to run away from the Walrider. Keep running until you climb several sets of stairs. At the top, jump across the gap and enter another decontamination room. Go up more stairs, and you’ll find the document on a desk in the room.

Note: Death of Hope

After a few cutscenes, you’ll be back in the room with Billy. Record him a second time to get the note (avoiding spoilers as much as possible).

Whistleblower DLC - Hospital - Notes & Documents

When you enter your very first vent in the DLC you will see two guards talking to each other record them

When you press the button you will see a doctor infront of you he will get attacked and record him dying

Document 1/5

Shorty after Note 3 When you run away from the Walrider you will enter a decontamination room after that you will see a doctor running away, follow him (he will run through a door) you need to enter the door on your right to get your document

Note 4/5

when you enter the area with the "Frank Manera" (you will know who he is trust me) Record him playing with the body

Document 2/5

When you pick up the handcuffs keys from the dead body, from that body go through the door off to your right and continue on in the next area and you will find your document

Document 3/5

Once you escape the furnace look for a door on your right, you will see an exit door in here to look right in this room and you will see some chairs and a altair at the end behind that will be your document

When you escape from the "Frank Manera" you will have to sqeese through some shelfs the next area will have a wheel chair on your left and have a door next to it enter the door and you will be in the toilets open the door and you will see a guy trying to kill a doctor so record him to get your last note

Document 4/5

From Note 5 continue on untill you see a decontamination room flooded with gas carry on and climb through the broken window look right and go through the door to find your document

Document 5/5

After another chase scene with the "Frank Manera" you will open a door to a lab looking room pick up the document on the desk

Whistleblower DLC - Recreation Area - Notes & Documents Locations

Note 1/2

As soon as you get outside head downstairs, as soon as you hit the bottom look right and you will see a door, enter the door and look up towards the window you will see Frank Manera again so record him to get your note

Note 2/2

Later on you will find some stairs you have a choice to go right or left, Go left (you cant go right anyway just yet) and go around the building you will see a locked door, you will also see a guy playing basketball with someone's head, (Behind that locked door) record him to get the note

Document 1/1

When you hit your second ladder in this outside area you will have to climb it, go inside the building when you get to the top and grab your document

Whistleblower DLC - Prison - Notes & Documents

When you see father martin writing on the wall record Him to get your note

From note 1 go down the hole and go in the first door you see, you will hear inmates shouting in that room so pick up your document

Whistleblower DLC - Drying Ground - Notes & Documents

after you ran away from chris walker in the last area, When you get outside again you will be on a rooftop from here drop down and look at the building that you was on and look for a window to climb through in the left corner of the building will be a document

when you crawl through some water, you will go up some stairs and straight in front of you, you will see an archway and some double doors next to that will be your document 2

When you turn the power off to the fence, when you get close to the fence again the power will turn back on you just need to record the fence coming back on

you are still outside, when you have to crawl through a hole in the fence door you cant miss this one so record the dead guy after he falls to the ground

Whistleblower DLC - Vocational Block - Notes & Documents

When you see a body hanging infront of you record the body.

Document 1/2

Shortly after note 1 you will need to squeeze through the gap and you will be attacked by the guy trying to get you through the wall, you will run into the room where you need to move the the object away from the door to continue ignore that and go into the next room to find your document.

When you head down in the basement you will run right past where you need to record, so record the naked dead body before moving on.

Note 3/5

After Almost losing the ability to have children, record the bench you was laying on to get your note.

After you Jump out the window to escape then on your right will be the document.

When you go through the vent in the building record the "Ceiling Decorations" (you will understand)

Whistleblower DLC - Exit - Notes & Documents

when you enter this area look out the window and you will see a burning a building record it to get your note

Note 2/3

From note 1 continue on untill you see some Guards they will be looking at body on the floor (Do You Remember Him?) so record them to get your note

From note 2. take the doors on your right, jump over the desk, take a left, then a right in the door in this room will have your document

later on you will run down a long corridor, take the door on your left, you will see a static tv go through the next door, when you leave take the next door on your right side and enter the room to pick up your last document of outlast

When you are about to leave mount massive asylum in the main lobby you will see a guy next to the door record him before you get close to him

the consul, collectables only count in one play (save file) so u need to collect it during one game. My camera recording was bugged and i tried to load checkpoint but it don't work. So i tried to load chapter but i didn't get achievement at the end... So you need to have all notes and documents in your "TAB" menu at the end of the game.
