I've got 100% based on his guide, but I just would like to add some details AND tips for others.
recomemend you that you read his guide first and check the tips if you have questions.

SPOILERS ALERT - beware it includes some spoilers!

Straight Flush

Flush every toilet - there are 3 toilets you need to flush, but somehow flushing them in different chapter didn't work out for me, no worries! you can flush all 3 in chapter 4. So don't worry if you missed in previous chapters. make sure you DO NOT forget to flush in Edie's drea.

Move On Out

Purge the mover's dream -
** If you've got ch.1,2 for free, it is possible that chapters disconnected between 2 and 3 when you purchase later chapters and continue your journey within. Make sure you have all chapters before proceed it.

For example, I got the card from mover in chapter 1, and after I purchased later chapters, I didn't have option to put helmet on the mover, it says "Victor can't do that, it might infect the mover". So I had to another run to get it.

If you called the mover in the Edie's dream in chatper 4, you accidentally infected the mover, therefore, you can access his dream and cut an extra tentacle, meaning, an achievement.

Thy Kingdom Come

Appoint a new king - you need to talk to three rats on the ship in chapter 6. The exact same holes that you placed rat poison in earlier chapter. I did not know how to talk to the rats, fear not! You need the shrinking gun to go through the holes. Go back to Martin's dream and get the gun first. After you talk to 3 rats and get them talk about their father, go to Vampire's house and find the father rat!

Deliver Unto Me

Circumnavigate yourself -
You need TWO balls of yarn to do so - one from Selma's house and the other in Martin's dream, which you used to cross the gap of dream (with twig). Use one to climb up to hospital bed and use the other to tie the fetus Victor and pull him out.

The Pumpkin Prince

Restore the pumpkin kingdom -
It took me a while actually, you need to solve problems of glorious pumpkin kingdom! But you can't proceed by just talking contineously to pumpkins. It seems to be only moving forward as you are proceeding with main story. So don't forget to come back and talk to pumpkins often. I talked to them more than 20 times as I was afraid to miss the achievement. You might not need 20 times, but surely it takes time.

I've played the right to the end of the chapter, talking with the pumpkins after every step of the way and still there is one pumpkin that has not solved his problem.
This little punk it going to end up in some pumpkin bread if he doesn't get with the program
