No, it can be missed. I believe you have to talk with Adelaide about the story she read recently (this option shows up in Case 3, if I remember correctly? When she is still mad about our drunk trip). During my second playtrought I didn't talk with her (neither visit publisher) and Miles didn't start the story, the option Adeilaide/Upton showed up very early.
About "Unsolvable" section, I have good news for speedruners: you don't have to blocked routes with witnesses to declare case unsolvable. As long as you get one suspect, you can wrap up case without point out the culprit (even if you get who is the real culprit). If you mention this in this guide, then I sorry, I couldn't find info about this.
...and yeah, you make a good point about people getting stuck (with the Casebook), so I added an extra note on that achievement in the *Important notes* section ;)
