Introduction
The achievements for this game were only added very recently, so the developers will likely tweak them a bit in the near future. Because of that, this guide might not always be 100% accurate, but I'll do my best to keep it up to date as things are changed.

If you have any suggestions for the guide, or questions on how to unlock something, please write a comment below! I should be able to respond to most comments within a day.

Achievements in this guide are sorted by the way they are unlocked; for example, all achievements that involve dying in a certain way are listed under "Death Achievements."

If the guide helps you out please give a thumbs up!
Respawn/Escape Achievements
These achievements are unlocked simply by respawning and escaping as certain classes.

Gear up, Boys!
Respawn as Nine-Tailed Fox.

This is pretty self explanatory, and likely one of the first several achievements you'll ever unlock.

CHAAOOOOS!
Respawn as Chaos Insurgency.

Same as the previous achievement, you'll probably get this one pretty quick just by playing the game.

It's Always Left, Brothers!
Escape as D-Class personnel.

This can be pretty difficult to unlock, particularly if you're new to the game. The best way to get it is by going solo, so you don't draw attention to yourself. If you can get a keycard, try to upgrade it to an O5 (black) keycard using SCP-914 in the Light Containment Zone, because it will allow you to open any door in the game. Using this card, you can open the door to the LCZ armory and get some weapons, which will make things a lot easier if you happen to encounter an MTF unit or a Facility Guard. Once you make it out of the facility, go through the doors next to MTF spawn and walk all the way to the end of the tunnel until you respawn as a member of the Chaos Insurgency.

Alternatively, you can skip getting weapons and try to negotiate with the MTF so they will disarm you and escort you to the exit, allowing you to become a Cadet instead of a CI member.

For Science!
Escape as a Scientist.

Similar to "It's Always Left, Brothers!," but arguably a lot easier since guards/MTF won't try to kill you. The escape point is in the same spot it is for D-class; through the tunnel next to the MTF spawn.
Warhead Achievements
That was... Close.
Cancel the Alpha Warhead detonation in the last 15 seconds of the countdown.

Pretty simple. Just wait in the warhead room in Heavy Containment until somebody on the outside activates the countdown, then cancel the countdown between 10 and 15 seconds. This 5-second interval is the only time you can cancel it, as detonation becomes inevitable when the clock falls below 10 seconds.

T minus 90 seconds...
Survive a successful Alpha Warhead Detonation.

Fairly easy to do. As long as you're in the Entrance Zone when the music starts playing during the countdown, you should be able to escape, because the exit gates will be open unless someone with a keycard has closed them since the countdown began.
Death Achievements
These achievements are unlocked by dying in a specific way.

Deep Fried
Die from a Tesla Gate burst.

Just walk up to one of the Tesla gates in Heavy Containment and let it electrocute you. It will kill most players instantly, but for some SCPs it might take several shocks to completely drain your health.

Look! I'm a Rocket!
Kill yourself with a grenade.

To unlock this, you have to be playing on a server where friendly fire is enabled, or else your grenade won't actually do any damage to you.

Newb
Die inside the pocket dimension.

Just let yourself get taken by SCP-106, and wait in the pocket dimension until your health drains to 0. You can also just go out one of the doors, but there's a small chance that you'll escape the pocket dimension and not die if you do this.

Gravity Works Here!
Die from a fall.

The easiest way to do this is by jumping off of the railing above the Chaos Insurgency spawn outside Gate A.

Is this your first time?
Let SCP-173 snap your neck.

The easiest way to unlock this is by playing as a D-class or a Scientist. You can pretty much just sit in one place anywhere in LCZ, and there's a good chance 173 will eventually show up to kill you. If you're quick enough, you can actually make it to his chamber before he even gets out, so he's almost guaranteed to kill you.

Involuntary Rage-Quit
Die while entering a rage as SCP-096.

This is likely the hardest achievement in the game. To unlock it, you have to be lucky (or possibly unlucky) enough to be killed as SCP-096 while you're in the animation that transitions you into your enraged state. This means the window for getting this achievement is only a few seconds, so you'll have to try and time getting enraged so it happens when your health is extremely low.
The easiest way to do this is by jumping off the railing above the Chaos Insurgency's spawn.

Wow, really?
Die within the first minute of the game.

Fairly self-explanatory. There are two really easy ways to do this, but since they both involve the Heavy Containment Zone, you'll need to spawn either as a Facility Guard or SCP 106/096.In the room with a three-way intersection and a closet with ammo on the floor, you can jump off the railings to die instantly. If you can't find this room, you can also run into a Tesla gate.
Combat Achievements
These achievements are unlocked in one way or another by killing other players.

They Are Just Resources...
Kill 50 D-Class's as a Scientist.

This is pretty difficult considering the SCPs often kill most D-class before MTF/scientists can, so it will take a long time to do. Fortunately, the statistic of how many D-class you've killed is tracked through all the games you play, so after enough rounds you'll inevitably unlock this one. You don't actually have to be a scientist for your D-class kills to count in this achievement. MTF kills work fine as well.

Pew Pew!
Kill 5 enemies in less than 30 seconds.

The easiest ways to unlock this are by playing as either the Chaos Insurgency or the MTF. If you're Chaos Insurgency, simply wait at the MTF spawn until a new wave shows up, and then slaughter them with the Logicer. It is far better than any weapons the MTF have at close range, so you probably won't have much difficulty in doing this if you take them by surprise. If you spawn as MTF, go wait outside CI spawn until you see the Humvee coming with a new wave, and then throw a grenade. If you time it well enough, you can instantly wipe out the entire wave of Chaos Insurgency as soon as they spawn.

Microwave Meal
Kill an SCP with the Micro HID.

The easiest way to unlock this by far is against SCP-096, particularly if he's alone, because he cannot attack you unless you look at him, and he is slow enough that it will be impossible for him to escape you if he hears you charging up the Micro. The charge-up animation on the Micro-HID takes around 10 seconds, so be careful not to look at 096 too quickly, because he might become enraged before you can fire at him.

NANI?!
As Class-D, kill a Scientist holding a keycard.

The easiest way to do this is by obtaining the LCZ pistol from the bathroom, SCP-012's chamber, or SCP-173's chamber.

Secure. Contain. Protect.
Successfully re-contain SCP-106

The easiest way to do this is by sacrificing somebody in 106's chamber and then pressing the button, but it's also possible to unlock simply by killing him with weapons. You don't even have to be the one to kill/re-contain him; as long as you're playing on a team other than the SCPs, you'll unlock the achievement when someone else does it.

If You Want Something Done Right...
Kill an SCP as a Scientist.

To unlock this one, you pretty much have to have the Micro-HID unless you're lucky enough to deal the finishing blow on an SCP that's already been damaged significantly by other players. You can find the Micro-HID in a locked room in Heavy Containment, or upgrade another weapon in 914 until it turns into one. The Micro-HID only has one shot, so make sure you use it wisely and keep the beam focused on whatever SCP you're trying to kill.

My Cure Is Most Effective...
Cure 10 people in a single game.

Coordinate with the other SCPs so they trap the people in LCZ in positions where you can easily cure Scientists and D-class, and make sure you're the only one doing any killing. You can easily get 5 or 6 zombies this way, and when the MTF/Chaos spawn you can easily get several more by hiding next to the checkpoint between the Entrance Zone and Heavy Containment. Note: this achievement is bugged, and you can get it by curing as few as 2 people. However, that number is just the lowest I've heard of, and you might get it with only 1 cure..
Item Achievements
These achievements are unlocked by obtaining/using certain items.

Friendship
As a Scientist, successfully upgrade your keycard alongside any amount of Class-D.

Use SCP-914 to obtain a higher level keycard while a D-class player is in the room, and this will unlock.

Overpowered!
Pick up an O5-Keycard.

If you start as a D-class or a Scientist, this one can be obtained pretty easily by upgrading a keycard with 914, although other people trying to steal your card might pose some difficulty. If you're afraid of this happening, it might be a good idea to hide in one of the airlock-type rooms for about 5-7 minutes until the crowd leaves 914.

... You Thinking What I'm Thinking?
Find any gun as a Class-D.

The easiest way to do this is by obtaining the pistol from LCZ. It has 3 spawn locations: the women's bathroom, the desk outside SCP-012's chamber, and the office at the top of the staircase in the chamber where 173 and 939 spawn. If you don't manage to get the pistol, but you have a keycard, you can upgrade that until you have armory access then get a gun from there. If you can't make it into the armory for whatever reason, but you can escape LCZ, use the elevator in Heavy Containment that leads to 049's chamber. If you have a keycard with even low-level armory access, you can open up a room in his chamber that contains an MTF rifle and a medkit.

The Tables Have Turned
Disarm an MTF

Unless you're really fast, by the time you make it out of LCZ as a D-class there's a good chance a wave of MTF will have spawned. If one hasn't spawned yet, wait in hiding for a bit until they enter the facility. Some will likely be killed by SCPs, and you can loot a Disarmer off of the body of a Lieutenant or Commander. Then all you have to do is catch a remaining MTF member unaware or get lucky enough to find an AFK MTF player at their spawn point outside. Note: disarming a Facility Guard will count for this achievement too!
Miscellaneous Achievements
Is This Thing On?
Broadcast a 'helpful' message via the Intercom.

This achievement is easiest if you're playing as either an MTF Commander or a member of the Chaos Insurgency, because they both spawn with keycards that can open the intercom room. Just hold Q to say something over the intercom while standing in front of it, and the achievement will unlock instantly.

Larry Is Your Friend!
Successfully escape from the Pocket Dimension.

Unless 106 has already captured a lot of people who have died in his pocket dimension, you'll have to unlock this achievement by pure luck, because the correct door for escaping it is entirely random. If you see an item sitting in front of a door and the door doesn't have a corpse inside it, you should go through it, because someone probably placed the item there before going in and escaping successfully.
Removed Achievements
Achievements in this section have been removed from the game for whatever reason, but it is possible they will be re-implemented in the future.

Roasted
Take some damage from a Tesla Gate as an SCP and do not die.

SCPs have high enough health that it takes two shocks from a Tesla gate to kill them, so you can just step into one briefly so it shocks you once then step back out. This is easiest if you're playing as 106, 049, or 096, because all of them spawn in Heavy Containment.
Removed as of July 5th.
Possible bugs
Here is a list of bugs/strange behavior you may or may not encounter as you try and unlock all of the achievements. If anyone experiences these or has something to add, please let me know!

-Achievements don't actually unlock until the game is closed after their conditions are met.

-The "cure 10 players in a single game" achievement unlocks even if you don't cure 10 people. The lowest I've heard of it unlocking at was 2 cures, but the requirement might require just a single cure to unlock.

-Killing D-Class as an MTF member or a Facility Guard will count for the achievement "They Are Just Resources" despite its description saying you need to kill them while playing as a Scientist.

-"Microwave Meal" (unlocked by killing an SCP with the Micro-UHD) might not unlock at all even if the game is closed.

-"The Tables Have Turned" says "Disarm an MTF," but it can also be unlocked by disarming a guard.

-"Larry Is Your Friend!" might not unlock when its conditions are met.

Sorry, I never ended up updating the guide with all the new achievements because an update in the last several months made the game essentially unplayable on my PC. My hardware is great, but whenever I tried to run the game it wouldn't go above 15-20 FPS. Things may have changed since then, so I guess whenever I have some free time I'll try running the game again to get those new achievements.

I will do the new achievements, but it’s going to be two or three days. A bit busy with school right now so it’s hard to find the time!
