Story Achievements- Home Sweet Home
The achievement "Home Sweet Home" is unlocked when you enter the Gamers' HQ for the first time.

To do this, you have to get your team's rank to at least 100. Gain rank by doing side missions and catching spydrones, spraying propaganda posters, etc.

Once your rank is 100 or higher, get the hacking key for the Sewers from Power Yellow, who is standing by the entrance. You will have to chase down the key and bump it 3 times.

After you have the key,you can disable the shield by the entrance by walking into it. Keep walking and you will load into sewers, then the achievement will pop.
Story Achievements- Re-Elected
To get the achievement "Re-elected" you simply just have to finish the game.

The bolded in white objective in the objectives box (top right in the hud) is the next thing you have to do to continue with the story.
Story Achievements- Revolution!
In order to get the "Revolution!" achievement, you have to collect all of the Gamegirls. After you have all of the gamegirls, do the final missions against the Great Admin again. Doing so will result in a secret ending and this achievement. You will also unlock the Breaker E-Cop DNA kit when you unlock the secret ending.

Tip: There are a few community guides on gamegirl locations, you can use those if you don't feel like finding all 206 of them yourself.
No Time To Waste!
The achievement "No Time To Waste!" requires a new character as this is the only way to get into the tutorial.

Create a new character in your team menu or on a new team. It will load into the tutorial. Quickly skip through all the text and run through the next few tutorial rooms. In the room after where you jump between the 3 platforms, run as fast to the hall on the left as possible. Go through there and bam, there you go. Just make sure to do this quickly as the door closes after 40 seconds.
Criminal
To get the "Criminal" achievement you have to have unlocked the prison through the main story.

This is a simple achievement- Get caught 3 times by E-Cops and Secuboxes without clearing your wanted level and you'll get thrown in prison. To get out, simply scan the blue thing in your cell and find your way back to the town.
Ravenous!
This is possibly the easiest achievement of them all and has no outside requirements. Simply find your way up to the More More More burger place in Haven and jump through the holographic burger.
Never A Moment's Rest
This achievement requires two people, so you must be in Friends Only or Online.

In the Administrative district, jump off of the tower in the middle of LazyTech Center place towards the giant Hypets. You can bounce there if you need to. When you land on a pet, it will bounce you upwards. Find your way into the panda's bowl (it won't bounce you out of there). Hang out in there, stand around, dance, graffitti, whatever. Have a second player do the same jump off the building, but this time they have to get on top of the panda. This will send you flying out of the bowl and the achievement will pop.
HAL 9000
In the Administrative district, by the E-Cop tower, there's a big reddish-white circle. Climb up to the ledge above it (it is best to bounce off of somewhere else to get up there). Then fall from the ledge and touch the circle.
Social
This is completely straightforward- Just participate in 10 different missions with at least one other player.
Happy Hypet
You need to get your hypet to the highest level of happiness (over 70% happiness). This is best achieved with an ultimate pet and squashoo flavor. 4-5 of the same tier chip (as long as they're the dirty or higher) in a pet e-swap will result in a squashoo. You can do this 10 or so times before it runs out too. General suggestion is 35 squashoo flavors before moving on.

Keep feeding your pet squahoos and it will eventually get to the highest level of happiness. Congratulations, you have the achievement now!

Finding my way back to this... I think, something that's (still) missing are pictures of the achievements.
