Persistent Upgrades / Perma-Perks
Persistent Upgrades were introduced with Tranzit/Green Run. They are often called Perma Perks, which is incorrect, because they are only somewhat 'permanent' under specific conditions. You gain all those Persistent Upgrades by fullfilling specific tasks and once gained you keep them in future games till you lose them. If you earn an upgrade you will see a green flash and the character will comment it.
With 'Die Rise/The Great Leap Forward' Treyarch added 3 more Persistant Upgrades, so it is not unlikely that we will see more added in the future.
'Fast Revive'
Quick Revive or Fast Revive is an upgrade which allows you to revive downed players faster. This upgrade will make the Quick Revive Perk less valuable, since you get the same effect without using one of your four 'slots' you have to obtain perks. The upgrade and the perk stack, so if you have both its like an instant revive.

You get this upgrade by reviving 17 times during a mp match. Since it happens rarely that you revive 17 players during a match, the best way to obtain it, is by creating a lobby with a friend and he will down himself (standing on lava or get hit by the last zombie) 17 times during the first round and you just revive him.

You lose this upgrade if you try and fail to revive a downed player. You will even lose it if the player get succesfully revived in a second try or by someone else.

According to COD-wiki the times for reviving are:

with neither: 3 seconds
with one: 1.5 seconds
with both: 0.75 seconds
'Persistent Jugger-Nog'
Persistent Jugger-Nog will make players go down after 4 instead of after 2 hits. This upgrade is pretty useful for the starting rounds and if you want to do the 'richtofen side' of the 'tower of babble' easter egg, since you have to do all the steps with power off.

The easiest and the only way i know how to obtain it, is by starting a solo game and buy quick revive and let yourself get downed. A green flash should appear and you now have the upgrade even in coop zombie games.

It seems like you will lose lose this upgrade when you either reach round 15, bleed out if you get downed or if you die from fall damage.
'Deadshot'
With this upgrade the zombie's heads will blow regardless where you hit them and it also increases the headshot damage. This is one of the upgrades i often lose and obtain within one game.

You gain this upgrade by making a collateral headshot. Best way is to train up a lot of zombies in a flat area and aim for the heads.

You lose this perk when you kill too many (according to wiki 25-75) zombies without headshots. I often lose it during an insta-kill phase, because i either just shoot 2-3 bullets into a horde of zombies or i just knife them to get the most points possible.

'Steel Barricades and Carpenter'
The steel barricades upgrade will make all boards you rebuild out of steel and it takes more time for the zombies to get the off the windows/wall. The newly added Carpenter will rebuild all barricades as steel barricades when picking up a 'carpenter' power up. These are 2 different persistent upgrades and you dont own both if you obtained one of them.

Steel Barricades
The steel barricades you obtain by repairing a large amount of regular boards within one game.

You lose this upgrade when you fail to build a single barrier in a round.

Carpenter
You gain this upgrade by killing a zombie while the boards from carpenter are being applied.

You will lose this upgrade when you fail to kill a zombie while another carpenter applies new boards.
'Red Insta Kill / Inst Kill Pro'
This Upgrade will kill every zombie which touch you during an insta kill phase. The upgrade lasts for 15 seconds and will fade out a bit earlier than the regular insta-kill.

You gain this upgrade by not killing any zombies while an insta kill is in effect, twice in a row. (as of now i havent been able to get it on pc)

You lose this upgrade when you get damaged by a zombie while the red insta kill is in effect.
Cash Back
This peristent perk will give you back points after purchasing perks. You gain this perk, if you lay down after purchasing perks and you lose it, if you miss laying down once. You dont obtain it after the first few trys and you need to have 100 perks obtained beforehand. Since i havent got it myself yet, cant tell if all of that is true, and how much points you get back for each of the different perk machines, but i have seen it in action and cant assure its a legit persistent perk.

Also, I'm pretty sure perma jug only works for low rank players. I think just the bones.

With the red insta kill, I haven't done it in a while, but I think that the way you loose it is actually by shooting (maybe knifing) a zombie while insta kill (Red and Reg) are in affect. During insta kill, you can ONLY kill zombies via red insta kill. I might be completely wrong tho.
