[THIS GUIDE IS VERY OUTDATED]
I wrote it in 2017 and the game was pretty different back then. I'm keeping this up more for archive now, but this guide does not represent the game well as of now.
SO YOU WANNA BE INFECTED?
So, you wanna be an infected, huh? Well too bad, you have no choice in the matter. Whether or not you wanted to be infected or not, you're stuck with it. So. What do you do?

Newcomers to the game will generally have a hard time playing as an infected player for many reasons.

1. They are too obvious. Alot of times these new players like to drink blood in the open, where people can see them, or are not aware of how loud the blood may be.

2. They are scared. They are scared of trying to go for blood for fear of being caught, so they just play as if they are innocent and try nothing. I'll be honest, I fell into this category my first few games.

3. They don't realize what infected can and can not do. For example, destroying traps or drinking blood, and instantly give themselves away.

4. They try to deceive the others...horribly. They are just terrible liars. Nothing really more to say on it.

So, what can you do to fix these problems?
BASIC BEGINNINGS
If this is your first infected game and you've not seen the tutorials, you're basically gonna be screwed.

So general idea of the infected players:

There are 2 infected every game, and you are one of them. You and your partner must work together to ensure all innocents die. The easiest way to do this is to transform during the blackout. What I mean by this, is that during the blackout, you are able to transform into your "terror form" and execute innocents. You can not save an innocent after they've been executed unless you have the antidote.

In order to transform, however, you must drink blood bags. These are scattered around the map, and only infected players can drink these. To transform, you have to have drank a maximum of 3 blood bags. However, when the blackout occurs, these blood bags only give half the amount of blood. So during the blackout, you would need 6 bloodbags provided you haven't already drank.

IMPORTANT: Infected players are able to use a seperate voice channel than the Innocents. However, they can use both. To use infected voice chat, hold down the V key by default. If the microphone icon in the top left turns red, that means you are talking in the infected chat. You can also do this just as games are starting, so you can figure out if you're infected before the game even tells you!

Co-ordinate with your infected partner and take 'em down!
TYPES OF TERROR FORMS
In the game, the transformed form of the infected players is called the "terror form". In this form, the infected player is able to "execute" players through a single click, provided they are able to get close enough.

In the game currently (patch v4.0.5) there are 3 Terror Forms to choose from.

Experiment, Yeti, and Werewolf.

The Experiment and Yeti are free for all players to use, however the Werewolf requires you to buy the Werewolf pack.

Well what do all of these different "Terror Forms" do?

First and foremost, all of the abilities for the terror forms are all activated by right click.

Experiment: This sends out a clone of the terror that runs forward for a few seconds. If the clone is damaged, it will disappear and leave smoke where it died. Personally, I don't know why you would want to use this one. It's ability seems pretty weak compared to the other 2, and seems to be easily counterable.

Yeti: When activated, the Yeti gains invulnerabilty to all forms of damage, including Light Damage. This makes him incredibly powerful, despite his slow movement speed. If you were to step in a trap with the Yeti, you can immediately undo all the damage the trap did by using his ability. This is my go-to for terror.

Werewolf: When activated, the Werewolf howls and disorients the innocents for a short time. Personally, I think this one is the most balanced of the 3. His ability isn't to hard to deal with as an innocent, and his fast movement speed makes up for that and his low health. He's fun to play too. (also I love the different skins it has)

IMPORTANT ITEMS
During the game, you may come across levers. These levers, once activated provide items for the
innocent to fend off against you. However, you may be asking yourself: "Wait, why would I get items as an infected? I have no use for them." However, that is wrong.

True, you don't necessarily use the items as they're meant to be used, but instead steal these items away from the innocent so that they cannot fight back against you.

Here's a list of the most important items against a terror:

Camera: This item can literally make or break your game as terror. Cameras in the game flash the infected when transformed, and make it so that the terror is slowed down (sometimes to a halt) and it makes the infected unable to execute when flashed. This right here can be what makes you lose. ALWAYS take this item if you have the opportunity.

Flashlight: It's basically a dumbed down version of the camera, but sometimes it can really screw you over. Just like the camera it can slow terrors, but using this opposed to the camera should be a last resort.

Antidote: ALWAYS ALWAYS ALWAYS MAKE SURE YOU HAVE THIS. The antidote can revive any player that has been executed (or lethalled) and if you take this item, they are at a MAJOR disadvantage. Also, make sure you keep this for the end-game, so that if your partner is lethalled (i'll get to it) you can revive them.

Inspection kit: This little gadget scans any player you want, telling you if they are infected or innocent. This thing will get you killed. Make it a point to always grab this if possible. Communicate with your infected partner and get it before innocents do.

Lethal Injection: This thing is great. It's essentially an insta-kill weapon that can only be accessed on the 3rd floor. Attacking someone with this puts them down, as if an infected executed them. The only way to revive them from this is the antidote. If your infected partner gets lethalled, ANTIDOTE THEM.

Traps: These really only come in handy near the end as an innocent. If you trap the exit gate/hatch at the end, you can almost ensure a win. This is because when the gates are opening the terrors can automatically transform. If you place traps before you open the hatch then you are set. If someone in your group is infected and transforms next to hatch away from the traps, then run through the traps so they trigger. If the terror camps the gate in this situation, just shoot them down.
However, as an infected, counterplay to this can be a little difficult, and could also require teamwork. An easy way to win against this is to have one infected destroy the traps while in human form and then have the other infected transform before hand and run in quickly while everyone is distracted by the person destroying traps.
One of the biggest things I see people doing with traps is placing them near bloodbags, and this is the biggest waste of the item. Traps only activate if you are already transformed, so this won't stop a terror from drinking it. And if you see a trap by a blood bag and you're ready to transform, of course you won't transform in front of it.
As an innocent ALWAYS ensure that you save your traps for the end game
As an infected, make sure you get these first so that they can't use them.
WAYS TO PLAY
Playing as an infected can be hard. You have to play mind games with the people you're playing with in an attempt to shift the blame onto someone else, or keep them in the dark entirely.

Talking about this is hard, because everyone is different, and so they are all going to play differently. I'll try going over the most common playstyles I see.

The Passive: This guy often chats with people about things in game or things unrelated to it. He's the type of guy you wouldn't expect being an infected unless you caught him red handed. Be wary.

The Silent: This guy never talks, probably because he doesn't have a mic. Sometimes you won't expect him, but most of the time he will get caught doing something suspicious. But surprisingly this dude tends to use his silence to his advantage, and can often win games. However, I can not stress this enough: GET A MIC IF YOU WANT TO PLAY THIS GAME. This game is all about communication and deceiving others through that communication. Without one, you will be screwed.

The Deceiver: This dude knows what he's doing. If you're against him, there's a good chance you won't find him out. He often communicates with his teammate, often placing blame on people, even saying things he did was actually done by someone else. This dude will screw your mind.

The Hoarder: This guy will make it a point to take EVERYTHING from the innocents, while at the same time making him seem innocent. If the innocents need an antidote? He already has it. If they want to scan with with the inspection kit? He already has it. This dude is the BIGGEST nuisance to an innocent team, and even innocents do this. This makes him seem suspicious, but not enough to immediately assume he's infected.

The Obvious Murderer: This dude probably gets caught red-handed murdering someone as infected. Whether it be he transformed in front of a group or de-transformed at the wrong time, he is usually found out. However, often these people are great at hiding from groups, so usually he can go on killing sprees all the time. Most of the time, this person has a 50/50 chance to win.

The Scared: This dude just chills with everyone else, and tends to not say anything that would give him away. He wants to drink blood, but he's too scared that someone would catch him in the act. I was like this when I first started playing, and if you want to be successful as an infected, DON'T play this way.

The Blamer: This dude won't stop saying it's someone else even if their life depended on it. They find a story, and they stick to it. Hell, some of them even claim their story is true even after being voted out. These people will often try to shift the blame onto someone else when they caused it. A coordinated team will win against this person easily.

The Obvious Blamer: This person is basically a mix between the blamer and obvious murderer. When they're found out they like to hide away, drink blood, etc while claiming that someone else was and has been doing it. Usually these people are never believed, but I've been in lobbies where someone believes this person when they drank in front of a whole group of people. You will usually win against this kind of person, but then again, it depends on how they play.

Out of all these playstyles, I personally am a mix between the Passive and the Hoarder.

There are also a few things I probably missed, feel free to list them in the comments.
TIPS AND TRICKS
Now that I've gone over some common playstyles, I'd like to teach you about certain tips and tricks to fool enemies as an infected. Some of these can prevent you from being heard when drinking, or completely shift an arguement about who is infected.

SHOOTING WHILE DRINKING: I don't often see this used, but I personally use it all the time. An easy way to cover up the sound of you drinking is to fire a shot as you drink blood. This paired with the perk "Quiet Quencher" can make you almost silent when drinking.

KEEP TRACK OF SURROUNDING PEOPLE: When playing as an infected, any time a player begins to run, the infected see an outline of that character. Use this to your advantage. Always. From there on, it's common sense not to drink when you see people in a nearby room, but try and distance yourself from the group when you go to drink. Try not to distance yourself too often, it's suspicious.

INSPECTION KIT TRICK: Something that is very commonly done is infected using the inspection kit. Why do they do this? The inspection kit is an easy way to take blame off, or shift the blame to someone. Inspecting your infected partner and saying "Alright, they're innocent." usually keeps people off of you. However, some people have caught on to this, so be careful. Another way is by scanning an innocent and saying "This one is infected, vote him." This works often, but if these people are in a group, then it can be hard to persuade them. Be careful, because if they have no reason to believe you, they will most likely gun you down.

BLUFFING: During the blackout when you transform, an easy way to persuade people is by saying "Someone transformed, get away." or something along those lines. Or, you could say "Guys, help he's chasing me!" Which could lead them off the scent for a while.

CALL OUT: When drinking blood bags, you could call out that there is blood missing. Some people become wary of you if you see blood missing but do not mention it.

SHIFTING THE BLAME: If someone sees you drink blood, a way to shift the blame is to flip it on them. Say that they drank the blood, and INSIST that they were lying. This doesn't always work, so only try and do this if people are already unsure of someone's innocence.

DON"T GIVE UP: (props to TheDeathSheep for this one) If you've been found out as an infected, do not give up. Once you've been found out, it will make the game harder for you, but you can still easily evade the innocent given you know what you're doing. Keep a close eye on the outline of players and try to stay away from the group. Hide in areas they won't be going, or just run around in a circle around the map, trying to stay on the opposite side of the innocent. Always make sure to have enough blood so that when the blackout does come, you can transform and make it easier for yourself.
WAYS TO COUNTERACT COMMON PROBLEMS
Finally, I'd like to go over ways to counteract the common problems I mentioned in the first section.

1. They are too obvious. An easy way to counteract that is to watch closely on the outline of players. If that doesn't help you, try not to speak unless you absolutely have to. Sometimes silence is helpful for you.

2. They are scared. Again, watch the outlines of characters. Take risks, it will benefit you.

3. They don't realize what infected can and can not do. Research this, learn about what automatically shows the innocents you are infected. Because if you are caught doing these things, it's game over.

4. They try to deceive others...horribly. If you are playing infected, and you feel like it's time to accuse someone, NEVER just randomly pick someone. You have to be watching what's happening, and what everyone is saying. Be cautious of who they are suspicious of, because you can use their distrust of that person to your advantage. This whole game is about lying. Lie to them in the most convincing way possible.
