NVIDIA Drivers
First make sure that you have NVIDIA drivers, and of course a NVIDIA Graphics Card.
If you have drivers downloaded, you should have a NVIDIA Control Panel.
* Right-Click on Desktop *
If you don't have it, go install it here.[www.nvidia.com]

Fixing the pixelated and blurry skins with Control Panel.
So now you have checked that you have installed NVIDIA Drivers, let's go to the Control Panel! Zooming the picture when doing these is recommended, also read the text I have wrote.

<- So it should look something like this.
You can see the steps in picture.
1. Press "Adjust image settings with preview"
It should go to the Adjust image settings with preview place.

2. You should see a bad looking NVIDIA logo with jagged edges. Pull the arrow all the way to the Quality. The NVIDIA logo should look really smooth and good

3. Press Apply at the bottom right next to the Cancel.

4. Wait it to load.

5. Restart / Start CS:GO.

6. ???

7. Profit.

Didn't that work? Let's try this one.
1. Press "Manage 3D Settings"

2. Press Program Settings. Then select CS:GO on the "Select program to Customize". If you can't find CS:GO, press Add next to it and select csgo and "Add Selected Program"

3. Then press the "Restore" button. And select "Yes".

4. Then press "Apply".

5. ???

6. Profit
Didn't the Control Panel fix it?

1. Try to put In-Game settings to Semi-High, or Ultra if your computer can. That can fix it.

2. If you have made some tweaks, try to disable them.

3. If you have made changes to video.txt, try to set them it to default.

My video.txt settings:

"VideoConfig"
{
"setting.cpu_level" "0"
"setting.gpu_level" "3"
"setting.mat_antialias" "0"
"setting.mat_aaquality" "0"
"setting.mat_forceaniso" "0"
"setting.mat_vsync" "0"
"setting.mat_triplebuffered" "0"
"setting.mat_grain_scale_override" "1"
"setting.gpu_mem_level" "0"
"setting.mem_level" "0"
"setting.mat_queue_mode" "-1"
"setting.csm_quality_level" "0"
"setting.mat_software_aa_strength" "0"
"setting.mat_motion_blur_enabled" "0"
"setting.fullscreen" "1"
"setting.defaultres" "1024"
"setting.defaultresheight" "768"
"setting.aspectratiomode" "0"
"setting.nowindowborder" "0"
}

Those settings can also prevent you crashing after map load. These will make your resolution to 1024x768. You can change "setting.defaultres" and "setting.defaultresheight" to make the resolution better. Those can also make some changes to the settings that you might not want, so paste those at your risk.
