It happens. I work as an OR circulator. While doing a urological case, we were at a point where a duel port catheter would be be inserted for continuous bladder irrigation after the procedure was over. The female scrub tech used the common slang asking "Do you want a 3-way?". The surgeon didn't even look up as he said "Sure, but I need the catheter first". Classic!
