Hey, from looking at your blender issue I know what's the cause: it has to do with part of the collision mesh being concave. It's dumb, but Source doesn't take too kindly to handling that kind of stuff.

Fortunately, though, there's a workaround to the mesh problem. All you would have to do is "chop" the mesh into small pieces. (Smaller slices = better concave mesh.) Select sections of vertices and use the 'Split' command.

Please let me know if this helps. I'll look into finding a supplement or two as well.
