As you seen above I had this same issue..solved..wipe everything..uninstall steamvr then unplug the hmd and usb..reboot..then uninstall all bluetooth drivers ..reboot..then go to appdata remove all steamvr files folders...then go into the steam folder and there will be folders there left for oculas and lighthouses..delete those ..then there are 4 files in the steam folder .vrconfig and some others open them with note and delete any with vr content. Then reinstall steamvr..reboot..then plugin your headset and start steamvr...go to setting and make sure to turn off powermanagment to steam usb devices...then reboot once more and it should be fixed:steamhappy:
