VAC is an automated system that detects cheats in direct use with your account, in association with family sharing a VAC protected game and cheating from either account, or on an account linked with the same phone number at the time of the infraction that was also VAC banned.
All this and more is covered in the pinned thread linked above, including pages linked within it. Thanks !
Correction...there is no evidence that SAM or achievement managers cause VAC bans. But like anything else, use them at your own risk.
SAM does NOT cause a VAC Ban

Malik's proof about SAM on this TF2 account
http://steamcommunity.com/profiles/76561198173473939/
If you watch the vids on Malik's TF2 profile that was linked, you can see SAM being actively used while connected to a VAC server. No ban and he says he has reported that profile and encouraged others to do the same. No VAC ban yet on it so that seems to prove it about as well as can be proved, I guess. Thanks.
