I would also never leave it running in any way while steam is running.
I probably wouldn't risk possibility getting a VAC ban just to cheat in an old pinball game, even if the chance would be low.
If you want to test stuff with less risk you can use http://virtualbox.org/ to run a virtual machine and cheat on that thing.
If you really want to install it, go ahead, but you assume 1000% of the risks involved with any bans that might come your way.

Since you are the one that didnt know anything about the VAC system, i will try to enlighten you, you dont get a VAC for just running Cheat Engine.........
And your friend was obviously lying, because cheat engine needs to be hooked onto a game process in order for it to do anything with it. I've had cheat engine running years ago while playing a vac-protected game. It was used for a different game and I had it open with tf2, no vac ban.
Consider how much it is worth for you to try just for this pinball thing and what you risk to loose if you screw something up or if someone here doesn't know everything (including me)
What you do know is for sure is what you would loose if you got VAC banned and what you are trying to do with this tool and how important you think it is for you.
For any other use of cheats regardless of server, remember your contractual obligations:
