I haven't come across many games that won't allow you to remap things to those buttons, unless it was one that didn't allow for mouse re-mapping at all.
Wonder what they're for...
It's up to you to decide for what they are to be used.
If it's a Logitech mouse, you can download the program SetPoint and configure those buttons to do whatever you want. That can cause some issues with games, though, so I suggest setting the default to be "Do Nothing" and then set application specific stuff.
For RTS I just map them to quick select buildings.
It really depends on where exactly the side buttons are. For instance, my Logitech M510 mouse has a nice, large rubber area just below the side buttons on which you can rest your thumb without having to worry about pressing either button.
It's so handy as a back/forward button (no more navigating to the corner of windows and browsers), but on the Razer Deathadder the mouse 5 is situated a bit too far forward for my hands to reach quickly during gaming sessions.
I think it's a combo of my hands being smaller than whatever the mouse was designed for, and the way I hold the mouse.
The Intellimouse Explorer 3.0 had both thumb buttons in the perfect spot for my thumb, but that mouse died on me, and last I looked they weren't as advanced as the Deathadder (maybe that's changed), so I went with the Deathadder for my new mouse.
I'll look into other mice next time
Also I like how I can bound keys to the mouse wheel left/right scroll
I had to use the side buttons for World of Warcraft, playing as a warrior needed 2 keys at the same time which was very annoying.
So I would be accidentally flashlighting allies in the face and taking swings at them with my chainsaw.
