Unfortunately you are not going to find a game to replace Ultima as it had a very unique appeal and history to it.
I've play UO as well as own Ultima 1-9. They were the best RPGs I had ever playes, crossing into several styles of play. UO was more action, Ultima 3-8 was strategic combat. Ultima 1 and two were simple and fun, where you just touch the monster. So much fun playing those games.
Mhmmmm...it is difficult to explain the "trends"...imagine an RPG game, with many many MANY skills, that define your character; those skills can be swordsmanship, parrying, archery, magery, necromancy, or crafting skills such as tinkering, mining or blaksmiths. You have bonification skills such as lumberjacking or anatomy that, depending on your gameplay style, can be pretty useful. You have many dungeons, a wide wide world to explore; housing system that includes (in some servers/versions of the game) the possibility to build your own. Banking system, a pretty unique PvP system. Great bosses, cities, factions. Clients supporting 2d/3d. Amazing overall.
That said, I am aware it is not possible to find a game that copies everything to the core and success, but maybe a game with housing system (think of it as skyrim's expansion), a 3rd person pvp/pvm system, Diablo 2-ish with different factors that can turn the fight, and many role possibilities, being able to do whatever you want.

its probably not exactly what your looking for but a good rpg experience and very open world with caves you dive into in search of loot with magic in one hand and a sword in another
