Report a translation problem
One could have confusion on what gender one wants to be. Or one could want to be special, like i'm not a male, I'm not a female, I'm something more.
I mean, if we had the freedom to change our body to suit our whims effortlessly, why settle on just one option with its strengths and weaknesses, when you want everything.

This is why they need way more representation!

while it is true that many games give the player tools to create a custom character via a character editor to allow them to create any character they look, that is something that only affects the player.
NPC's don't get a character creator to change into whatever they want.
so, to stay true to the whole thing about the world and setting being believable, its not unreasonable to portray the one or the other character who may be trans or whatever. they aren't a player character so they don't get access to a character editor and depending on the setting, maybe they simply can't afford whatever magic scrolls or sci-fi genetic alteration would allow them to transition into who they are.
while this is unfortunate for the character in question, this can provide story opportunities because it creates conflict and struggle.
whether you like that or not is something you gotta decide

I welcome any sidequest that isn't the usual copy/paste "go to Z, kill X amount of Y and then come back to me" kind which has become prevalent in many games.
But this all boils down to the root of someone's ego and incepting the thought that one can be born in the wrong body, which is the ultimate nonsense and delusion peopel are sadly struggling with.
You have that one body and if you are not liking it then start going to an life coach.
People rather divorce than repair a broken marriage, it's very "cool" nowadays to change your sex when you are not satisfied with the one you were born with.
It's just the next step after surgery extremism was not enough.
