You rinse the lemon to remove any pesticide residue that was not washed off (washing is done by the farm, after picking). Any wax on fruit is food grade wax and safe for consumption, but can also be easily washed off as well.
You can add any fruit to enhance the flavor. There are also other things people have added, such as cucumber and/or mint leaves.
This actually applies to all water+Fruit combinations but as OP said Lemons are by far the most common.
I just use lemon juice or lime juice. Easier.
