Yes. As long as the game is running, the time counts as in-game time.

I don't think it does as i had 120 hours in a game and left it up and running while i took a nap came back 8 hours later and closed the game out it was only at 121 for the hour i played. Not the 8 hours i slept and had the game paused.

i remember when i didn't turn off my pc when it was runnning a game and it went into sleep mode and logged some hours anyway

Well the game can tell if you don't make a mouse move or keystroke... but it usually ( for online games ) will boot you off the server + game back to the main menu... but even some games count that as hours in if not played.
