I just drove by on the sidewalk on the wrong side of the road at 100MPH, destroyed the trafficlights and lightposts, made people jump into the street and get hit by other cars, all while a cop car was sitting waiting for the now destroyed light to change that just fell on said cop car, and they do nothing.
Sheesh, at least have them give me a ticket.

I "tapped" a single pedestrian with my car, cops murder 10,000 people and destroy half a city trying to catch me by driving like nutjobs and shooting all over the place in seemingly random directions with no regard for civilian safety, sometimes even calling in tanks and helicopters with miniguns.

Inept guards.
Playing "stealth" games... Kill a guard around another guard, they may raise an alarm or go into high alret for about a minute, then suddenly its like nothing ever happened and they just go back to doing their thing with their dead guard buddy still there.

Locked doors in shooter games.
I have a freaking grenade launcher and chainsaw, but most wooden doors are apparently invincible.
Why'd they even put it there?
For that matter, anything that should be easily damaged/breakable but ISN'T.

Women have giant boobs but men in tight pants have no "bulge" what-so-ever.
I guess all the musclebound guys in games do steroids so much that they have major "shrinkage" down there when they squeeze into their tight pants/bodysuits.

If you are going to make armor realistic/practical for men then at least do the same for women.
I'm pretty sure "Joan of Arc" didn't go into battle in a chainmail bikini.
And if you are going to make armor sexy and impractical, at least make it sexy for the guys too.
If a woman is going into battle in a chainmail thong, give the guys the option of going into battle in a chainmail speedo... If anything it'd be for the luls and eyecandy.

"Achievements"... In general.
Like the "Congratulations! You opened a door on the first tutorial level, in a linear corridor that wouldn't allow you to progress without opening said door!" achievement.
Seriously, until achievements actually "DO SOMETHING" they are nothing more than footnotes pointing out how little of a real life the player has.
No such thing, save for a few tiny caliber "spyguns" usually in the subsonic under .22LR size.
Putting a suppressor on a gun makes it anything but silent, all a suppressor does is slightly muffle or reduce hearing damage from firing a gun so you don't need to wear ear protection and it "muffles" the report of a firearm from long distances.
For that matter...
"Shooter games where the main character isn't stone deaf from firing guns unsuppressed without hearing protection"
There should be a lot more "WHAT DID YOU SAY?!?!?" and "I CAN'T HEAR YOU OVER THE RINGING IN MY EARS!" and "WHO?!? WHAT?!?!"
When can we play as the villain?!
