There isn't really much in the way of anything else that could cause this.

I just used the test call option in skype and the following happend to my speaker icon in the bottom right corner of my screen.

If I click on the speaker icon it opens up a window that start searching for solutions for an audio problem and the only way to get my speaker icon back to normal and get my audio back is by closing down steam and skype which I just had to do.
Note: this issue also happens when calling through steam itself, I have yet to test teamspeak but I expect the same problems.
Headset is a turtle beach X12.
Next time u have Skype open, left click on that Speaker icon again and click Mixer. Then see if any Volume levels for other open apps are changed. When u have multiple apps open (even Games) u can set each to their own volume with that mixer.
Then go to: Communication -> Do nothing.
Win8, 8.1, 10 are the same way.
