What is rep?
I will occasionally see a "+rep" comment on someone's profile. What does this mean?

+rep is a crude add to reputation. It means, most likely, that whoever traded with him was satisfied.

Also don't expect people to have only good reputation just because their profile comments, they could remove any -rep comments.

+ scammers often use it to create a sense of reliability before the scam

In addition to removing negative comments, certain people also ask their acquaintances to post +rep on their page without having traded anything whatsoever!

I've seen people go that far as to 'rep' themselves, by commenting on their own account to fake being a 'reputable' trader. Sad as true.

As you say those "+rep" messages are something not to be trusted when considering someone as a trusted trader.
Yup, scammers do use it. One account of mine was scammed by this guy, and on his profile there is a +rep by my scammed account.

+rep means nothing at all.. you can just delete -rep and all that... so its just useless

Tradeing to the rules and not for keys is one of the better ways to avoid scammers the stemrep site seems to help as well. but comment on a stem prfile - not so much as others have said they can just delete stuff they don't like.

So does it actually make a difference, like on Steam is there actually something that keeps track of when people +rep so you can see say a users "score" by using all the + and - reps and generating a score? or is it just out of common courtesy to +rep a nice trader?

There is no track of it, and that is good as users will have abuse it if there was, over all Rep Is a useless thing that is used by scammers to confuse new users
