Why is this happening all the time?

Ya, it happens. If the items don't show up after the trade is failed on either side then just restart the client. That happened to me once and I freaked out.

Absolute rubbish.

Firstly the servers rarely go down. It's just congestion - it's been VERY busy of late. The only thing I can offer is to try again at a less busy time, or offer the trade so that it is seen by the person next time they log in (ie. not a "real time" trade).
