This is no different to the claims from people who say "xxx game broke my Xbox" - it just isn't true.
Your graphics card and it just HAPPENED to be during that game.
I wouldn't think it has died though, as I've seen those green lines when the card has either become unseated correctly, or is new and there's residue on the pins.
You might want to try removing the card, and cleaning the pins with isopropyl alcohol on a cotton bud (be careful to ony moisten the bud).
But games DO NOT kill cards in general. That's nonsense.
Then follow the instructions I laid out. Then report back.

And how have you worked this out?
Did you try the alcohol cleaning as I asked? I stated before to FOLLOW MY CLEAR INSTRUCTIONS.
1. Took off my GPU and cleaned it out
2. Reinserted it correctly and plugged in the pins correctly, nothing happened.

1. Plugged in different monitors, nothing happened
2. Unplugged my GPU and repositioned the monitor cable from GPU to motherboard and was able to access the PC smoothly
3. Downloaded the latest drivers, installed them, and plugged the GPU again and repositioned the monitor cable back to the GPU
4. Kept getting BSOD so I switched to Safe Mode, monitor goes "No Input Mode"/"Not on Optimized Mode" (different messages due to diff. monitors) and goes on standby mode.

This was the end of the line
1. Unplugged my GPU and repositioned the monitor cable from GPU to motherboard and was able to access the PC smoothly
3. Uninstalled all my AMD drivers and downloaded Linux to test GPU's boot capabilities
4. Installed Linux on a flash drive and turned off my PC
5. Switched Boot Mode to my flash drive
6. There were tons of artifacts from the Boot Mode window and Windows logo to the Linux boot window itself
7. Selected "Start Linux" at Linux boot window and hit Enter
8. "No Input Mode"
9. Monitor went on standby
Did you do as I suggested and use isopropyl alcohol (tape head cleaner, if you don't know what it is)?
If you just did it dry or with something like water, then it's likely to have done nothing.
Of course, your card MAY be fried, but until you nail down and eliminate the options properly you'll never be sure.
Make sure also that you REALLY push the card in firmly, as I've seen this issue before when a card is very, very slightly out of spec (size wise). Had a friend who just couldn't get rid of these artifacts, despite cleaning and the steps you suggested, but when I opened it up, I took it out, couldn't see anything wrong, but it was very tight going back in until it gave a very firm "click" (which my friend said he'd never pushed it that far before) and al worked swimmingly.

By all means try again.
However, do bear in mind that oxidiztion builds up over time so you can have such an issue at ANY time.
I'll just warn you again to be extremely careful with the alcohol - don't get it near anywhere else on the board. Just make the cotton bud moist. Don't be afraid to rub reasonably firmly. You can't hurt the gold contacts. You can hurt the circuit board though.
If this still shows no improvement, then I'd say you've done about all you can and it does look likely it's dying or dead.
EDIT: Final thought - can you try your card in another slot in your PC?
Sigh, guess I'm gonna have to wait until next year to get a new one. Just my rotten luck.
