These are assuming defaults:
SpeedTest and ISPs show speeds in Mbps (Megabits per a second)
Steam shows speeds in MBps (Megabytes per a second)
8 Mb = 1 MB
So 91 Mb on speed test should show as about 11 MB on Steam. That is really close to the max speed and with in a reasonable difference.
I don't know about Speedtest, but on Steam:
Steam > Settings > Downloads
Ensure that "Display download rates in bits per a second" is toggled to (blue).
Your steam downloads are limited by
1) your cpu
2) your disk IO
3) your anti-virus
4) your ISP
pick one
Steam doesn't throttle on their end.

It could be any number of issues from the servers, routing, ISP throttling or IPv6 issues.
I don't know what to do since I literally didn't do anything.
I downloaded Helldivers 2 from France to the east coast of the USA at 400mbps
Steam isn't throttling anything. Its on your end
It wasn't on my end after all. It was an issue with a few of Steam's servers.
Note to this actually more on your ISP end where it routes poorly depending on your destination. Steam doesn’t throttle based on location. If it did I wouldn’t get 400mbps from France to the USA
I mentioned routing.
Now to determine if it was Valve's routing or the ISPs.
