Close DS4Windows.
Enter Bluetooth devices Devices and remove the wireless controller.
Load DS4Windows
Make sure hide DS4windows is checked in the options
In Bluetooth devices add a new devices
Put new DS4 in pairing mode
Once connected it should be fine.
If in the log it says it couldn't open in Exclusive mode disconnect the DS4 and connect it again. May have to repeat it 2/3 times before it connects and doesn't have exclusivity issues.
Once sorted make sure DS4windows is loaded before you load Steam. If you load Steam first or DS4windows has issues using Exclusive mode Steam, and DS4Windows will constantly fight for controller control.
Please note:
DS4windows is not needed for Steam with either gen 1 or gen 2 DS4 controllers. You go into big picture and uncheck all boxes except for DS4.
Secondly DS4windows was written for DS4 gen 1 controllers only. Inputmapper by the same creator works with both gen 1 and gen 2 DS4 controllers.
I use Steam input for DS4 when using Steam and boot inputmapper up for all other gaming clients.
In recent months many users have had their controllers stop working for no reason, or work erratically, and so on.
The only ways around this I know of are to try pairing in Big Picture mode, or just using the cable instead of bluetooth (which would probably be the best solution, as bluetooth with Sony proprietary stuff is ALWAYS spotty). At a real stretch you could try using another utility like Joy2Key to get some functionality out of it, or better still Xpadder or InputMapper.

Did you reset the blue DS4 before testing on the PS4?
And what about Xpadder and InputMapper - have you tried those?
